Objectives: Psoriasis(psoriasis)is a common chronic and recurrent disease.The course of its onset-remission-reoccurrence is throughout the patient's life,and can be accompanied by arthritis,atherosclerosis,hypertension and other organ damage and metabolic disorders.The disease is complicated and can not be cured.At present,the treatment field of psoriasis is chaotic,and the prevention and control of psoriasis has been transformed from a single biomedical model to a new biological psycho social medical model.The effective intervention and management of the course of psoriasis is an urgent problem to be solved.The data information management system for psoriasis patients is established to realize the batch storage of patients with psoriasis,collect the individual and disease information of psoriasis patients efficiently and comprehensively,carry out deep data mining on the mass information,carry out the early risk factors of psoriasis and its associated disease,and change the passive psoriasis treatment.It should be an active management of psoriasis,improve psoriasis efficacy,prevent recurrence and improve the quality of life of patients.Methods: Use psoriasis specific disease database,namely psoriasis patient data information management system.The system contains basic data of psoriasis,which is easy to use and fast,and is the main data acquisition system.According to the diagnostic criteria of PV and GPP,psoriasis patients were selected from January 2017 to December 2017 in a certain skin disease hospital of a class three class a general hospital by using the data information management system of patients with psoriasis.The first group selected 320 PV patients and 320 normal controls,second groups of 60 GPP patients and 60 PV patients.The general situation(medical history,physical examination)and laboratory examination information(blood cell parameters,blood lipid five items)were collected and analyzed at the baseline.Using SPSS17.0 statistical software,the measurement data conforming to normal distribution are expressed with mean mean standard deviation(x + s),and the percentage of counting data is expressed as percentage.T test,single factor variance analysis(ANOVA),minimum significant difference method(LSD),Bonferroni method and Kruskal-Wallis test are used in different groups.Spearman correlation analysis method is used to express correlation between indexes.Reslts: Part one:Application of data information management system for psoriasis in psoriasis vulgarisUsed the data information management system of psoriasis patients,320 cases of PV patients were collected and compared with the control group for general situation,blood lipid parameters and blood cell parameters.The following results were obtained: the patients with BMI more than 24 in group PV accounted for 50% of the total number,indicating that most of the PV patients in the group were obese.There was no significant difference in the BMI between the two groups(P=0.077),but when the range between 24 and BMI was less than 32,the number of obese patients in the PV group was significantly higher than that in the normal control group.The score of the area and severity index of psoriasis in 320 patients with PV(Psoriasis area and severity index,PASI)score was 5.7-46.8,with an average of(21.8±8.23)points.The dermatology life quality index(DLQI)score was 1-26 points,with an average of(11.30±4.88)points.204 cases(63.75%)were progressive stage lesions.There is a significant correlation between PASI and DLQI(r=0.889,P=0.000).Among the five blood lipids,the lipid metabolism in group PV was abnormal,and the P value of TC,TG,HDL-C and ApoA had significant statistical significance(P < 0.000),and LDL-C value was not statistically significant(P=0.367).The parameters of blood cell were as follows:(1)the parameters of red cell: RBC,MCH and MCHC were statistically significant(P > 0.05),the ratio of MCH and MCHC had a high statistical significance(P=0.001,P=0.007),HB,RDW,HCT,MCV.(2)the parameters of white cells were both significant.The statistical significance(P=0.000,P=0.003),LYM,NEU,BAS and EOS were not statistically significant(P > 0.05);(3)the platelet parameters: MPV,PCT and PDW P were less than 0.05,and PLT values were not statistically significant(> 0.05).In this study,320 cases of PV patients were divided into the blood hot wind type,blood deficiency wind dryness type and blood stasis skin type according to the characteristics of TCM syndrome type.The proportion of three types was 41.56%,37.19% and 21.25% respectively.There was no statistical difference between the sexes in type 3(P=0.22).When compared with blood deficiency group and blood stasis group and blood stasis group PASI,no statistical significance was found(P=0.17,P=0.30),and the PASI value of blood deficiency group and blood stasis group had a high statistical significance(P=0.000).In the age of the disease,the blood stasis group was significantly higher than the other two type,with a high statistical significance(P=0.000),and the age of the blood heat group was slightly higher than that in the blood deficiency group(P=0.045).The blood stasis group BMI was higher than the blood deficiency group,the difference was statistically significant(P=0.000),compared with the blood heat group(P=0.037),when the blood heat group and the blood deficiency group two groups were compared,the BMI value was not statistically significant(P=0.59).Blood lipid parameters: the TG value of blood heat group was higher than that of blood deficiency group,the difference was statistically significant(P=0.047).When TG was compared between blood heat group and blood stasis group,blood deficiency group and blood stasis group,the difference was not statistically significant(P > 0.05);LDL-C was significantly higher in blood stasis group and blood deficiency group,the difference was statistically significant(P < 0.0167),T No significant difference was found in C,HDL-C and APOA-I between groups(P > 0.05).Blood cell parameters: the comparison of HB,LYM and MPV between blood heat group and blood deficiency group was statistically significant(P < 0.05),while the comparison between blood heat group and blood stasis group found that the P values of HB,EOS,LYM and PCT were less than 0.05,and the NEU values of the blood deficiency group and blood stasis group were higher,the difference was statistically significant(P < 0.016).7),compared with blood stasis,the number of EOS in white blood cell parameters was statistically significant(P=0.006),and the NEU value was statistically significant(P < 0.0167).Part two:Application of data information management system for psoriasis in generalized pustular psoriasis :60 patients with GPP and PV were screened in psoriasis data management system.Group GPP: male 37 cases(61.67%),female 23 cases(38.33%),the ratio of male to female was 1.61:1,10-73 years old,with an average of(37.63±17.41)years.In group PV,there were 36 males(60%)and 24 females(40%),with a male to female ratio of 1.50:1,aged 14-64 years,with an average of(41.07±14.33)years.There was no statistical difference in gender and age between the two groups(P > 0.05).In group GPP and group PV,the ratio of BMI over 24 was 61.67% and 55% respectively.There was no significant difference in BMI between group GPP and group PV(P > 0.053).The relationship between PASI and DLQI: the correlation coefficient of PASI and DLQI in group GPP was r=0.891,the correlation coefficient of PASI and DLQI in group PV was r=0.731,PASI and DLQI values were significantly higher than those in the group,the difference was significant statistically significant.Blood lipid parameters: the serum levels of TC,HDL-C and LDL-C in group GPP were lower than those in group PV,with statistical significance(P=0.000,P=0.005,P=0.004),TG and APOA-I was not statistically significant(P > 0.05).Blood cell parameters: GPP group RDW,WBC,NEU,PLT value increased significantly,the difference was statistically significant(P < 0.005).Conclusion:1.The rate of BMI > 24 in both psoriasis vulgaris patients and patients with generalized pustular psoriasis was 50% or more,increasing risk factors associated with obesity,cardiovascular system and / or metabolic related diseases.2.Most of the patients with PV and GPP suffered from advanced lesions,with moderate severity of lesions.In the statistical study,the correlation between PASI and DLQI was found in GPP patients and PV patients,indicating that the skin lesion of psoriasis was positively related to the quality of life.PV type of Chinese medicine is mainly composed of blood heat and wind,blood deficiency and wind dryness type two.The age and BMI value of patients with blood stasis skin type were all higher than those of blood heat wind type and blood deficiency wind dryness type.The hemoglobin content of the patients with blood heat and wind was significantly lower than that of the other two groups,and the activity of inflammation was more obvious than that of the other two groups.Abnormal lipid metabolism was found in 3.PV patients or GPP patients,and the degree of lipid metabolism in GPP patients was more severe than that in group PV.4.The partial blood cell parameters of the patients with GPP were different from those in the patients with PV.The leukocyte count,neutrophils count and platelet count in GPP patients were higher than those of the PV patients,indicating that the patients with GPP had higher levels of immuno inflammatory activity.
