| Objective: By observing the effect of Yiqi Yangyin and Huatan Tongluo Fang on the clinical efficacy of coronary heart disease(unstable angina pectoris)combined with type 2 diabetes in patients with PCI,evaluate its clinical efficacy and application value, and explore its possible mechanism. Methods: By collecting the September 2015-September 2017 visit to the Department of Cardiology at the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine, newly diagnosed patients with coronary heart disease with unstable angina and type 2 diabetes and scheduled to undergo coronary angiography.Divided into observation group(standardized western medicine + Chinese medicine)and control group(standardized western medicine treatment)according to the principle of randomization, the corresponding inclusion criteria, exclusion and rejection criteria were established, and all patients were selected to meet the inclusion and exclusion criteria.A total of 75 patients were included in the initial screening.2 patients refused to implant stents,4 patients stopped Chinese medicines midway, and 7 patients were excluded after 6 months due to failure to review angiography.Eventually,62 patients were selected and 31 patients in each group. The general conditions of the two groups(including gender, age, history)and related clinical factors were comparable. Results: 1.Comparison of TCM syndromes: After treatment, both groups had significant improvement in symptoms of traditional Chinese medicine(P<0.01).Overall,the observation group was superior to the control group(P=0.025).At the same time, the observation group was superior to the control group in improving all symptoms of patients(P<0.01),showing the unique advantages of Chinese medicine.2.Comparing the effect of angina pectoris: The symptoms of angina pectoris were relieved in both groups after PCI.The observation group was significantly better than the control group(P=0.003),indicating that the observation group was able to improve the symptoms of angina earlier than the control group. After one course of treatment, the angina symptoms were further improved and the effective rate was further increased(P=0.046 in the observation group and P=0.003 in the control group),but the observation group was superior to the control group(P=0.046).3.Comparison of the efficacy of hs-CRP,BNP and cTnI: Both cTnI and hs-CRP decreased slightly after PCI,but there was no significant difference between the two groups before treatment(P>0.05).There was no significant difference between the two groups(P>0.05),but they could be observed from the histogram. The decline and trend of the group were more obvious, showing the potential advantages of traditional Chinese medicine in reducing cTnI and hs-CRP.After one course of treatment, the three groups of indicators showed a significant drop in comparison with postoperative and preoperative comparisons.Statistical significance was significant(P< 0.01),except for cTnI,the difference between groups was significant(P<0.01).The cause of cTnI no statistical difference(P> 0.05)of the reasons may be cTnI after treatment were reduced to the normal range, while the normal reference value range is smaller, and the two groups of data have smaller fluctuation range between 0-1.4.Electrocardiogram,LVEFcomparison: Afterthe intervention, the electrocardiograms in the two groups improved, with statistical significance(P<0.01).After one course of treatment, there was statistically significant difference between the two groups(P<0.01).It shows that both the intervention itself and the drug treatment can improve the ECG,and the efficacy of the combined Chinese medicine is significantly better than that of the western medicine alone.5.Comparison of CTFC: The distribution of LAD,LCX,and RCA before the first interventional treatment was comparable(P>0.05).After successful PCI and drug treatment, the reexamination showed significant improvement in the blood flow of the three major vessels. There was significant difference between the two groups. The significance of learning(P<0.01)was significantly better in the observation group than in the control group. Conclusion: Yiqi Yangyin and Huatan Tongluo Fang can significantly improve the symptoms of angina pectoris complicated with type 2 diabetes.Reducing cTnI,hs-CRP levels after PCI,alleviating myocardial ischemia, and improving electrocardiogram can reduce the incidence of perioperative complications. Can significantly improve the level of myocardial perfusion after intervention, reduce BNP levels, improve LVEF,help improve heart function, improve patient quality of life. Therefore, the combination of Western medicine standardization and intensive treatment based on traditional Chinese medicine can significantly improve the clinical efficacy, improve the prognosis, and safety is good, worthy of clinical promotion. |
