| 1 ObjectiveBy observing the effects of acupuncture and regulating traditional acupuncture by Western medicine and conventional medical treatment on ischemic stroke in patients with ischemic penumbra,the effect and efficacy of the study were evaluated,explore the optimal plan and some mechanisms of acupuncture treatment for patients with acute ischemic stroke.2 MethodsForty patients with acute ischemic stroke(AIS)who met the inclusion criteria were randomly divided into a control group(20 cases)and acupuncture group(20 cases).The acupuncture group adopted Tongdu Tiaoshen acupuncture and acupuncture in combination with routine western medicine treatment.The main acupuncture points selected Shuigou,Shangxing,Baihui,Dazhui,Yamen,Fengfu,Jingjiaji,Jinsuo,and Yaoyangguan.treatment once a day,needle for 40 minutes,needle for 1 time in 20minutes;the control group was only treated with conventional western medicine;both groups were treated for 4 weeks and 4 weeks for one course of treatment.Observe the effect of the two methods on the scores of NIHSS,mRS,and ADL Barthel scales before and after treatment in the two groups of patients.DWI technique was used to observe the relative apparent diffusion coefficient(rADC)values of the central area,the infarct marginal area,and the corresponding healthy area of the infarct focus before and after treatment;at the end of the course of treatment,the data before and after treatment in the same group were compared and the data before and after the treatment in the two groups were compared,the analysis shows that there are differences between the two sets of indicator data,analyze the results,evaluate clinical efficacy,and draw conclusions.3 Results3.1 rADC value comparison of two groups Before the treatment,there was no significant difference in the rADC values between the infarct central area and the rADC values in the infarct marginal area,and in the infarct marginal area in the same group(all P>0.05).Between the two groups and the same group,there was no significant difference in rADC between the two groups(P>0.05).The neuronal cells in the central area of the infarct can't be rescued;rADC values in the infarct margin of the control group were higher than those before the treatment(P<0.05).The rADC values in the infarct margin of the acupuncture group were significantly higher than before treatment.Compared with the control group,the rADC values in the infarct marginal area were significantly different(P<0.05),suggesting that both treatment methods can increase the rADC value in the infarct margin of AIS patients,and the combination of acupuncture and western medicine in AIS patients has a better rADC value in the infarct margin compared with routine western medicine treatment in patients with AIS.This suggests that acupuncture attunes can save AIS patients' IP and have better results.3.2 NIHSS score comparison of two groups There was no significant difference between the two groups before treatment(P>0.05),indicating that the two groups were comparable;after one course of treatment,there was a statistically significant difference between the two groups before and after treatment(P<0.01),suggesting that both treatment methods can reduce the NIHSS score;acupuncture group compared with the control group to reduce the NIHSS score is more obvious,the difference was statistically significant(P <0.05),suggesting that the acupuncture group has better treatment than the control group.3.3 Comparison of mRS scores between the two groups There was no significant difference between the two groups before treatment(P>0.05),indicating that the two groups were comparable;after one course of treatment,there3 Resultswas a statistically significant difference between the two groups before and after treatment(P<0.01),suggesting that both treatment methods can reduce the patient mRS score;acupuncture group than the control group to reduce the mRS score after treatment is more obvious,the difference was statistically significant(P <0.05),suggesting that the acupuncture group compared to the control group treatment is better.3.4 Comparison of ADL Barthel Index Scores in Two Groups There was no significant difference between the two groups before treatment(P>0.05),indicating that the two groups were comparable;after one course of treatment,there was a statistically significant difference between the two groups before and after treatment(P<0.01),suggesting that the two treatment methods can increase the ADL Barthel index score;acupuncture group compared to the control group increased ADL Barthel index score is more obvious,the difference was statistically significant(P<0.05);suggest acupuncture group than the control Group treatment is better.3.5 The two groups compared the total efficacyCompared with the overall clinical efficacy of the two groups after treatment,the total effective rate of the acupuncture group was significantly better than that of the control group,and the difference between the two groups was statistically significant(P<0.05).It indicated that the acupuncture group had better treatment effect than the control group.4 Conclusion4.1 TongDu Tiaoshen acupuncture is an effective method for clinical treatment of acute ischemic stroke,which can effectively improve the patient's clinical symptoms and prognosis.4.2 TongDu Tiaoshen acupuncture treatment of acute ischemic stroke patients achievesthe therapeutic effect of stroke,and its possible mechanism is related to benign intervention in the penumbra of patients. |
