Objective(1)To evaluate the clinical efficacy of Xiaoyu Ji San hot compress combined with arthroscopic debridement in the treatment of patients with knee osteoarthritis in the early and middle stages.The relationship between the content of IL-1 beta in the knee joint fluid and the degeneration of knee joint in patients with knee osteoarthritis in the early and middle stage was further investigated.(2)To further clarify the curative effect,mechanism and advantages of the treatment of osteoarthritis of knee joint in the early and middle stages by removing stasis and radiating heat.To find a more effective,reasonable and safe treatment plan for the treatment of knee osteoarthritis in the early and middle stages of the clinic.MethodAll the patients were in the hospital of the First Affiliated Hospital of Anhui University of traditional Chinese medicine(First Affiliated Hospital),the method is to observe the clinical effect,according to the inclusion and exclusion criteria of Pathology,a total of48 medical records were selected,and the 48 cases were compiled into 1-48 in sequence.According to the principle of randomization,they were randomly divided into the experimental group and the control group,with 24 cases in each group.In addition to receiving arthroscopic debridement,the experimental group also needed two boxes of removing blood stasis and bone setting and heat dissipation therapy,once a night for 3weeks,while the control group was only treated with Xiguan arthroscopic debridement.The experimental group and the control group should be strictly in accordance with the requirements of aseptic operation of synovial fluid and the use of antibiotics to prevent infection,AIMS(arthritis impact index)index and VAS score,were followed up for 3months for the first week and the fourth week after surgery and stay take synovial fluid by AIMS index and VAS score record to assess the curative effect of the treatment,the.The data were analyzed and analyzed with SPSS24.0.Result(1)AIMS index After 1 weeks of treatment,the experimental group AIMS index(49 + 7.36)than before treatment(62 + 0.97)was significantly reduced(P<0.05);after 4 weeks of treatment and before treatment(62 + 0.97)compared to the experimental group,the AIMS index(47 +5.42)was significantly reduced(P<0.01),the control group the AIMS index(92 + 13.93)than before treatment(96 + 2.91)slightly reduced(P<0.05);the experimental group after 1 weeks of treatment and after 4 weeks of treatment,the AIMS index was(49 +7.36),(47 + 5.42),there were significant differences between(P<0.05);we were 3 after months of follow-up from patients,the AIMS index of the experimental group(45 +5.75)than before treatment(62 + 0.97)was significantly lower(P<0.01),while the control group,AIMS index(85 + 14.05)than before treatment(96 + 2.91)decreased(P<0.05).It shows that the treatment of Xiaoyu Jie Gu San hot compress combined with arthroscopic debridement for early and middle stage osteoarthritis patients can significantly reduce the impact of arthritis on patients’ life.(2)VAS pain score After 1 weeks of treatment,the experimental group VAS pain score(4.6 + 1.22)than before treatment(5.6 + 1.22)was significantly reduced,the difference was statistically significant(P<0.01);after 4 weeks of treatment,the VAS pain score(2.7 + 1.28)before and after treatment(5.6 + 1.22)compared was significantly reduced(P<0.01);the experimental group,after 1 weeks of treatment and 4 weeks after treatment VAS pain scores were(4.6 + 1.22),(2.7 + 1.28),there were significant differences between(P<0.05);the control group after 1 weeks of treatment,the VAS pain score(5.5 + 1.10)after 4 weeks of treatment with VAS the pain score(3.8 + 1.28)slightly reduced,the difference was statistically significant(P<0.05);we were 3 months of follow-up in patients after the experimental group VAS pain score(2.1 + 0.78)significantly lower than before treatment,while the control group VAS pain score(2.7 + 1.43)than before treatment The two groups of data were statistically significant(P<0.01).It is indicatedthat the treatment of Xiaoyu Jie Gu San hot compress combined with arthroscopic debridement for early and middle stage osteoarthritis of the knee can significantly reduce the pain of patients with knee osteoarthritis.(3)The concentration of IL-1 beta in the articular fluid of the knee joint The knee joint fluid of experimental group and control group after 1 weeks of treatment IL-1 levels were(758.83 + 74.28),(972.88 + 10.76)than before treatment in knee joint fluid IL-1 levels were(971.59 + 38.50),(976.11 + 2.20)was significantly reduced,with statistical significance Yi(P<0.01).The knee joint fluid of experimental group and control group after 4 weeks of treatment IL-1 levels were(516.87 + 37.55),(709.85 +50.54)compared with the knee joint fluid before treatment in IL-1 beta level were decreased(P<0.01);synovial fluid of experimental group and control group after 1weeks of treatment IL-1 beta level(758.83 + 74.28),(972.88 + 10.76)and 4 weeks after treatment in knee joint fluid IL-1 levels(516.87 + 37.55),(709.85 + 50.54),the difference was statistically significant(P<0.05);the level of the experimental group in knee joint fluid of IL-1 beta after 3 months of follow-up(212.35 + 37.96)compared with before treatment,the difference statistically significant(P<0.01).We can see from the data that both the experimental group and the control group can reduce the level of IL-1 beta in the knee joint,but the experimental group is more significant than the control group.Conclusion The knee joint debridement has obvious effect on the treatment of patients with osteoarthritis of the knee in the early and middle stages;The effect of Xiaoyu Jie Gu powder combined with arthroscopic debridement in the treatment of early and middle knee osteoarthritis is remarkable,and it is superior to single knee joint debridement;The level of IL-1 beta in knee joint fluid in patients with osteoarthritis of the knee in the early and middle stages has a certain relationship with the degeneration of the kneejoint.Through a series of comparative data found that the effects of Xiaoyu Jiegu San fomentation combined with arthroscopic debridement in the treatment of early and mid-term outcomes of patients with knee osteoarthritis,is "Xiaoyu Jiegu San clinical curative effect mechanism of a further research,provides a theoretical basis for the use of the latest anti stasis jiegusan,and provides the effective and reasonable treatment for therapy in patients with osteoarthritis of the knee.
