Object:With peoples more and more attention to oral health,the treatment of caries has become one of the most regular situation people face.However,common dental instruments and methods used often results in the patient's fear,especially for pediatric patients.The sound,vibration and cooling water of high-speed dental handpiece are the main causes of dental fear.The development of chemomechanical method by Carisolv(Ya Dian)solves this problem in some degree.This procedure does not require the high and low speed handpiece,will not cause significant vibration,heat and noise.On the other hand Carisolv can remove the infected dentine selectively,to avoid excessive damage to healthy dentin and reduce the possibility of pulpal exposure(1).In this study,we compare chemomechanical method by Carisolv(Ya Dian)with traditional mechanical method from the efficacy of caries removal and influence on filling.We want to define the clinical feasibility of Carisolv(Ya Dian)through HE staining experiment,microleakage experiment and micro tensile test.Methods:29 extracted molars with large area of caries deep to dentin were collected.Every sample was evenly divided into two halves before caries removal by traditional mechanical method and chemomechanical method of Carisolv(Ya Dian).We do the following experiments:(1)HE staining experiment(n=6).After completion of dental caries removal,the roots were cut and consecutive decalcified slices(4?m)were sectioned through the cavities.Then we examine the presence of affected dentine tubules under the microscope after the staining,the remaining dentin tubules.Comparing the difference of two groups.(2)Microleakage test(n=10).After completion of caries removal,filling the cavities with resin and performing thermal cycling test.We compare the depth of microleakage after fuchsin staining.(3)Microtensile test(n=13).The caries were removed and filled before samples made into dumbbell-shaped slices with a resin-dentin surface of 1 mm2.The tensile force at break was measured using a micro-tensile dynamometer.Calculating the micro tensile strength and doing statistical analysis.(4)Recording the time comsumed by caries removal through two methods and comparing the difference.Results:(1)There was no significant difference in the the remaining dentin tubules between the two groups(P=(P=0.292>0.05).(2)There was no significant difference between the two groups in microleakage(P=0.059>0.05).(3)There was no significant difference in the microtensile strength between the two groups(P=0.363>0.05).(4)The Wilcoxon rank sum test showed that the decay time of the two groups was significantly different(P<0.05).The time consuming of Carislov was significantly longer than mechanical group,which was consistent with clinical application.Conclusion:The efficacy of caries removal,filling microleakage and microtensile strength of Carisolv chemomechanical method are similar to traditional mechanical method.Considering the advantages in comfort and behavior management despite the longer operation time,this is still an available method for treatment of caries.
