Objective:To investigate the level of fasting plasma lipopolysaccharide(LPS)in individuals with different with different glucose tolerance and body mass index(BMI)and explore its influential factor and the relationship among LPS,BMI and insulin resistance.Methods:ninety-nine subjects were recruited from the department of endocrinology and metabolism.The individuals underwent 75g oral glucose tolerance test(OGTT)and were divided into the normal glucose tolerance(NGT)and the type 2 diabetes(T2DM).According to BMI level as 28kg/m2,the two groups were also classified into obesity subgroup and non-obesity subgroup(NGT-OB/NGT-nonOB:19/22;T2DM-OB/T2DM-nonOB:28/30).Plasma LPS,serum high sensitivity C-reactive protein(hsC-RP)and some biochemical parameters were detected.The insulin sensitivity was evaluated by homeostasis model assessment of insulin resistance(HOMA-IR).Comparisons between groups were performed using independent samples T test or Mann-Whitney U test.The relationship between L PS and other variables were presented by the Pearson correlation coefficients.The stepwise regression model was used to analyze the influence factors of the LPS level and the HOMA-IRResults:The mean LPS level of OB subgroups in NGT group and T2DM group are as two to three times higher than non-OB subgroups respectively(NGT:5.01±1.94 vs.13.19±3.34 pg/ml,T2DM:8.1±2.03 vs.22.01±5.53 pg/ml),and are diagnosed as metabolic endotoxemia.(2)The plasma LPS level was positively correlated with BMI(r=0.307),HbA1C(r=0.454),hsC-RP(r=0.254),alanine aminotransferase(ALT)(r=0.881),aspartate aminotransferase(AST)(r=0.848),HOMA-IR(r=0.509),fasting plasma glucose(FPG)(r=0.450)(P<0.05).(3)Multiple linear regression analysis demonstrated that the LPS plasma was influenced by BMI,HbA1C and that BMI and LPS were the important factors influencing HOMA-IR.Conclusion:With the increment of BMI,the level of plasma LPS was gradually elevated,which made human body in low-grade inflammation and metabolic endotoxemia status and aggregated insulin resistance.
