Objective:In order to study the roles and correlations of TGF-β1 and PLGF in the malignant transformation of hydatidiform mole,to analysis the roles of TGF-β1 and PLGF in the development of hydatidiform moles,and the high risk factor relations of TGF-β1 and PLGF in malignant transformation of hydatidiform mole,to provide a reference method and theoretical basis for early prediction of malignant transformation of hydatidiform mole,a Immunohistochemistry method was applied to detect the expression of TGF-β1 and PLGF in early pregnant chorion and hydatidiform moles.Methods:The data of hydatidiform mole patients who received and treated in Yan’an University Affiliated Hospital was collected.All patients were followed-up by 1 year.We found that among those patients,31 cases were malignant transformed to GTN(30 cases were invasive hydatidiform mole and 1case was chorioepithelioma).We consider these cases as the malignant group.A simple random number method was adopted to select 33 cases of hydatidiform mole mild proliferation group as a no malignant group.33 cases of chrionic villus samples from the same period patients with normal pregnancy but requires abortion were set as a control group.The completely clinical data of three groups,paraffin samples of the first curettage data and chrionic villus samples from the normal pregnancy data were collected.A SV0002 two-steps Immunohistochemistry method was applied to detect the expressions of TGF-β1 and PLGF.The high risk factor relations of TGF-β1 and PLGF in malignant transformation of hydatidiform mole were analyzed.A SPSS 22.0 statistics software was applied to analysis all of the results.Results:1.The positive stain of TGF-β1 located in the cell membrane and cytoplasm of syncytiotrophoblast and cytotrophoblast.The positive rates of TGF-β1 in each group were: control group 97.0%(32/33),no malignant group 87.9%(29/33),malignant group58.1%(18/31).2.The positive stain of PLGF located in the cell membrane and cytoplasm of syncytiotrophoblast and cytotrophoblast.The positive rates of TGF-β1 in each group were: control group 36.4%(12/33),no malignant group 63.6%(21/33),malignant group 90.3%(28/31).3.Patients with high risk factors of hydatidiform mole than those without high risk factors,the malignant transformation rate were significantly increased.4.In hydatidiform mole patients whose age>40,h CG>10~5U/L,and the size of uterus > gestational age,the positive rate of TGF-β1 were 55.5%(10/18),63.2%(24/38),64.1%(25/39).In patients without above high risk factors,the positive rate of TGF-β1 were 80.4%(37/46)、88.5%(23/26)、88.0%(22/25).Patients with high risk factors of hydatidiform mole showed a lower positive rate of TGF-β1 than those without high risk factors.5.In hydatidiform mole patients whose age>40,h CG>10~5U/L,the size of uterus > gestational age,and thecalutein ovarian cyst diameter>6cm,the positive rate of PLGF were 94.4%(17/18)、84.2%(32/38)、84.6%(33/39)、86.2%(25/29),which showed a significant augment in those without the above high risk factors(the positive rate of PLGF were 65.2%(30/46)、57.5%(15/26)、56.0%(14/25)、69.2%(22/35))6.The expression of TGF-β1 and PLGF in no malignant group and malignant group was moderately negatively correlated.Conclusion:1.When compared to the hydatidiform mole without a malignant transformation,the malignant transformation hydatidiform mole showed a decrease in the expression of TGF-β1,while the expression of PLGF showed an increase.2.The hydatidiform mole patients who have high risk factors showed a decrease in the expression of TGF-β1,while the expression of PLGF showed an increase.3.The expression of TGF-β1 and PLGF in hydatidiform mole tissues was moderately negatively correlated.4.The decrease in the expression of TGF-β1 and increase of PLGF is an early event in hydatidiform mole malignant transformation.TGF-β1 and PLGF are involved in the malignant development.We can combine the high risk factors in hydatidiform mole malignant transformation as a reference in the prediction of hydatidiform mole.
