Purpose:1. To investigate the morphological characteristics of villous blood vessel of hydatidiform mole (HM) and normal early gestation.2. To explore the expression of vascular growth factor such as vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) in villous stroma of HM.Methods:Vasculature was examined immunohistochemically in the villous stroma in 14 cases of normal early gestation placental villi ,27 cases of partial hydatidiform mole(PHM) and 11 cases of complete hydatidiform mole (CHM).The stain intensity and distribution of CD34, VEGF and bFGF in these placental villi sample were evaluated via image analysis technique.Results:1. The microvessal density (MVD) in CHM group was statistically significantly lower than that of PHM group (p<0.01) .The MVD in PHM group was statistically significantly lower than that of normal early gestation group (p<0.01) .2. Antibody CD34 was able to detect the vasculature. Molar villi were stained by CD34. In the stroma of almost all molar villi examined, vessels were found, but their density and structure varied greatly in each molar villus even in the same CHM, and the vessels were irregular in diameter,In fully dilated molar villi the vessels often became narrow.Generally MVD was inversely proportional to the grade of the swelling of villi. No significant differences in vasculature were found in molar villi at different gestational ages.3. The content of CD34 in CHM group was evidently lower than that of PHM mole group (p<0.01) . The content of CD34 in PHM group was statistically significantly lower than that of normal early gestation group (p<0.01) .4. In villous stroma, the content of VEGF and bFGF in CHM group were statistically significantly lower than that of PHM group (p<0.01 and p<0.01, respectively) ; the content of VEGF and bFGF in PHM group were statistically significantly lower than that of normal early gestation group(p<0.01 and p<0.01, respectively) .Conclusions:1. Considerable vessels could be detected by CD34 in the villous stroma of CHM.2. The lower expression of CD34, VEGF and bFGF in villous stroma might have relation with the defect of vascularization in HM.3. The disturbance of blood development of HM might cause fetaldeath.
