Objective:To investigate the difference of clinical features for chronic obstructive pulmonary disease(COPD)patients with or without lung cancer(LC).Analysis of the clinical characteristics of COPD complicted with lung canceer,in order to improve the awareness of lung cancer,early diagnosis and treatment.Method:Clinical data of 50 patients with COPD combined with lung cancer hospitalized in the second affiliated hospital of dalian medical university between October 2013 to October 2016 were retrospectively reviewed,and 60 patients with only COPD were taken as the controls.They were compared mainly in terms of the following factors: gender,age,smoking status,clinical symptoms,imaging findings,pulmonary function classification,inflammatory index,coagulation and fibrinolysis index,arterial blood gas and tumor markers were compared and ananlyzed.The clinical data were analyzed using the SPSS 17.0.Result:1.The ratio of male to female was 7.3:1 in COPD patients combined with lung cancer,and 1.5:1 in patients with only COPD.Two groups gender compared in terms of gender,it was statistically significant(P<0.05).2.COPD patients combined with lung cancer aged 44 to 88 years old,the average age were(69.02±10.20)years;the age of the patients with only COPD was 48 to 85 years old,the average age were(70.40±10.50)years old.Two groups compared as regards age,there was no statistically significant difference(P>0.05).3.There were 45 smokers in COPD patients combined with lung cancer,41 cases of smoking indexs(the number of cigarettes smoked per day multiplied by the numberof years of smoking)?600;There were 44 smokers in COPD patients combined with lung cancer,35 cases of smoking indexs ?600.The number of smoking and smoking indexs of COPD patients combined with lung cancer were higher than those of patients with only COPD,and the differences were both statistically significant(P<0.05).4.Two groups of patients with cough,expectoration,fever and debilitation were compared,the differences were not statistically significant(P>0.05).The incidences of hemoptysis,chest pain,weight loss,dyspnea,hoarseness,pleural effusion and atelectasis in COPD patients combined with lung cancer were higher than those in patients with only COPD,and the differences were statistically significant(P<0.05).5.The proportion of lung function?in patients with COPD combined with lung cancer was significantly higher than that in patients with only COPD,the proportion of grade III or IV was significantly lower than that of patients with only COPD,and the differences were statistically significant(P<0.05).6.The differences of hs-CRP,N and NLR between the two groups were not statistically significant(P>0.05),but the PCT and ESR of patients with COPD combined with lung cancer were higher than those of patients with only COPD,the differences were both statistically significant(P<0.05).7.The levels of FIB and D-Dimer in COPD patients combined with lung cancer were higher than those in patients with only COPD,and the differences were both statistically significant(P<0.05).8.The differences of Pa O2 and HCO3-between the two groups were not statistically significant(P>0.05),but the levels of Pa CO2 in COPD patients combined with lung cancer were higher than those in patients with only COPD,and the differences were both statistically significant(P<0.05).9.The difference of SCC between the two groups were not statistically significant(P>0.05),but the CEA,NSE and CYFRA21-1 of patients with COPD combined with lung cancer were higher than those of patients with only COPD,the differences were both statistically significant(P < 0.05).10.Male,smoking indexs ?600 and ESR increase are the risk factors of lungcancer.11.Among the 50 cases of COPD combined with lung cancer,there are 24 cases of squamous cell carcinoma,18 adenocarcinoma,and 8 small cell lung cancer.There was no statistically significant sex difference in each pathological type(P > 0.05).Conclusion: 1.The inflammation index including PCT and ESR PCT and ESR,coagulation and fibrinolysis indexes including FIB and D-Dimer,tumor markers including CEA,NSE and CYFRA21-1 in COPD patients combined with lung cancer were significantly higher than those of patients with only COPD.2.Logistic regression analysis showed that male,smoking indexs ?600 are the risk factors of lung cancer.
