Objectives:According to report serum albumin(Alb)levels at admission indicate the metabolic disorders of the tumor and has a prognostic value for different cancers(gastric cancer,breast cancer,ovarian cancer,colon cancer,blood disease,etc.).In recent years,the incidence of gastric cancer in China has been increasing,and there is little report of serum Alb on the prognosis of young patients with gastric cancer.This study was to investigate the relationship between serum Alb levels and clinicopathological features and the effect on DFS in young patients with gastric cancer by analyzing the serum levels of Alb,the clinicopathological features and the postoperative disease-free survival(DFS)Prognosis.Method:According to the United Nations World Health Organization by the quality of the human body and the global average life expectancy and GBD2000 used in the report stratification,defined as young people under 44 years old.So,a retrospective study was performed of young patients with gastric cancer(YGC,44 years of age and less)who were diagnosed at Liaoning Province Cancer Hospital from February 1,2006 to August31,2015 and 59 young gastric cancer patients were enrolled thought inclusion and exclusion criteria.They were divided into two groups according to the levels of Serum Alb normal(<40 g/L group and ? 40 g/L group)by our hospital exam department.The statistical analyses were performed with the SPSS statistical package(SPSS Inc.,Chicago,IL,USA,version16.0).Clinicopathological features and levels of Alb were compared by chi-square test(Pearson chi-square,continuity correction,Fisher's Exact test).Pearson's Dependency coefficient analysis related effect size.disease-free survival(DFS)was evaluated using the Kaplan-Meier method and log-rank test.Univariate analysis and multivariate analysis was performed using the Cox proportional hazard analysis.Statistical significance was defined as p < 0.05.Result:The data of this group showed that serum Alb was in the range of 27g/L-49 g/L and the average preoperative serum albumin level reached 42.59 g/L.The levels of serum albumin <40 g/L group and ? 40 g/L group were compared with age,sex,tumor infiltration,lymph node metastasis,tumor site,TNM stage,CEA,CA19-9,blood type and smoking,drinking and other clinical and pathological features were not related.Univariate Kaplan-Meier analysis revealed a significant correlation between low Alb level poor disease-free survival.univariate and multivariate analysis showed that the depth of invasion,serum globulin,clinical stage,serum albumin and CEA were the risk factors associated with the prognosis of YGC.Furthermore,low Alb level and depth of invasion were independent marker for poor clinical outcome of young gastric cancer patients.(P = 0.015 HR = 0.0892,P = 0.032 HR = 3.815).Conclusions:The serum albumin level in young patients with gastric cancer was an important index for DFS of young patients with gastric cancer(YGC).The clinical and pathological factors of YGC were not significantly correlated with serum Alb levels.The results of this study serum Alb have a certain reference value to predict and evaluate the prognosis of YGC.Currently nutritional therapy is a popular topic in the field of cancer treatment,which is an important adjuvant therapy to prolong the survival of cancer patients.Decreased serum albumin suggests poor DFS and simultaneously Alb is a good indicator of nutritional status in patients.Continue to explore the serum Alb reduction in YGC at the time of admission can improve the DFS of YGC by takingnutritional support treatment actively.The prognosis of YGC are closely related to clinical factors,the degree of tumor infiltration,tumor staging,hematological indicators: serum albumin and globulin,CEA.And the depth of tumor invasion and serum albumin concentration are independent prognostic factors for the prognosis of YGC and serum albumin is the only protective factor.The risk of recurrence and metastasis of T3-T4 patients was 3.024 times that of T1-T2.
