Objective:To observe the changes of perioperative nutritional status and immune indices in children with perforated appendicitis,discuss the effect of early enteral nutrition on the immune function of children with perforated appendicitis after operation.And to explore the feasibility and clinical value of early enteral nutrition in children with perforated appendicitis.Methods:This study enrolled 70 patients,who diagnosed as perforated appendicitis,receiving emergency laparoscopic appendectomy in general surgery of Shanxi Province Children's Hospital.The children were randomly divided into early enteral nutrition group(n=35)and control group(n=35),both groups were given routine treatment.Immune indexes(IgA,Ig M,IgG,LYM)and serum nutritional indexes(albumin,pre-albumin,hemoglobin)were compared between two groups on day 1 before operation and day 1,day 7 after operation.The two groups were compared in terms of length of postoperative hospital stay,total hospitalization cost,and postoperative complication.Results:Patients' characteristics,severity of illness,nutritional status(PA,Alb)and immune indices(IgA,IgM,IgG,LYM)were similar on admission between the two groups.Compared with the control group,notably higher level of PA,Alb,Ig A,IgM,IgG,LYMwere observed in the EEN group,nonetheless,the level of Hb was similar in both groups on days 7.Conclusion:Early start(24 hours after surgery)of Oral enteral nutrition is safe and effective.Furthermore,it can help to restore the gastrointestinal function,improve nutritional and immune status,thus reducing the occurrence of postoperative complications.
