Objective:In this study for renal medicine hospital CKD2-4 period of dialysis patients basic information,on the basis of investigation,through carries on the management mode of intervention itself,which discuss the self management mode in CKD2-4 the application result in dialysis patients and make an evaluation.Methods:This study surveyed from January 2017 to June 2017 Tai Yuan city 3 armour hospital urology department two units CKD2-4 period of dialysis patients in hospital general information and basic condition,it can be divided into control group and experimental group and control group given routine nursing intervention,the experimental group on the basis of conventional nursing care,give self management mode of intervention,by comparing before and after intervention in patients with renal function index,nutrition index,KDQOL-SF,rating scale change,analysis of self management mode for CKD2-4 period of dialysis patients quality of life and the influence of disease progression.Results:(1)this study surveyed 88 CKD2-4 hospitalized patients,46 patients with the treatment group and control group patients with 42 cases,42 patients completed the final experimental intervention(4 cases dropped out),the comparison of two groups of patients with general information,there is no significant difference between difference(P > 0.05),both groups were comparable.(2)Before-and-after itself: the experimental group patients after the intervention compared with before intervention,the patient's blood urea nitrogen,serum creatinine,systolic blood pressure,body mass index(BMI)were significantly decreased(P < 0.05),hemoglobin and diastolic blood pressure mean no difference(P> 0.05);After intervention,patients' KDQOL-SF scale scores increased significantly in all dimensions(P < 0.05),and the difference was statistically significant.The mean values of systolic blood pressure and body mass index of patients in the control group decreased after intervention compared with those before intervention,and the difference was statistically significant(P < 0.05).There were no statistically significant changes in serum urea nitrogen,serum creatinine,hemoglobin and diastolic blood pressure(P > 0.05).The KDQOL-SF scale score of the control group showed no difference after intervention compared with that before intervention(P > 0.05).(3)Contrast between groups: 6 months after the intervention group patients blood urea nitrogen,serum creatinine,systolic blood pressure and body mass index was significantly lower than control group(P < 0.05),hemoglobin levels have no obvious difference(P > 0.05),diastolic blood pressure compared with the control group after the intervention no difference(P > 0.05).After the intervention group and control group in the SF-36 scale scores for the eight dimensions: physiological function,social function,physical function,emotional function,energy,and general health of six dimensions scores were significantly higher(P < 0.05),the physical pain and mental health of two dimensions scores no difference(P > 0.05).KDTA rating scale: in the experimental group in the symptoms and discomfort,life,life burden,social quality,sleep,medical encouragement,satisfaction,social support,nine dimensions of cognitive function score was significantly higher than that of control group(P < 0.05),and in working condition,and sexual two dimension score no difference compared with control group(P > 0.05).Conclusion:The results showed that the application of self-management mode management of ckd2-4 non-dialysis patients could effectively improve the quality of life of patients and delay the progress of renal diseases.
