Research On The Effect Of Kin Supervision In The Volume Management Of Peritoneal Dialysis Patients

ObjectiveTo explore the effects of the kin participation in health supervision on the patients with peritoneal dialysis(PD), and evaluate the result on patients’ scores of volume knowledge questionnaire, chronic disease self-efficacy scale and volume management behavior scale and clinical index.MethodsThis was a experimental research. 85 patients were randomly divided into intervention group(43 cases) and control group(42 cases) by using random number table in the PD center in a top three hospital. Patients in the control group were only treated with routine health education. Patients in the intervention group were conducted with kin participation in health supervision. The forms of intervention included collective education, telephone follow-up, network education. The contents of intervention were basic knowledge of capacity management, the clinical index,actions to deal with sudden volume overload. The study lasted for 6 months. Before intervention, after intervention, the data was collected, and the clinical index data was collected before intervention, 3rd month intervention period and after intervention for evaluating the effects. The statistic methods included descriptive statistical analysis,t-test, chi-square analysis, repetitive measurement and analysis of variance,generalized estimating equation.Results91 patients undergoing PD were selected conveniently, 6 cases were lost for kidney transplant, transfer to other hospital, hemodialysis, 85 patients who completed the entire study were divided into intervention group(43 patients) and control group(42 patients).1. The results of t-test showed that there were statistically significant difference(P<0.01) in patients’ volume knowledge, self-efficacy, and volume management behavior in the pre-post test.2. The clinical indexes of the patients with capacity management were analyzed by repeated measures analysis before and after the intervention, and the results were as follows(except edema degree):(1) The difference in weight was statistically significant between groups(F=6.246, P=0.014), time effect was statistically significant(F=24.226, P<0.001),and there was interaction effect between time and group(F=24.094, P<0.001).(2) The difference in systolic pressure was no statistically significant between groups(F=1.593, P=0.210), time effect was statistically significant(F=235.067,P<0.001), and there was interaction effect between time and group(F=134.225,P<0.001).(3) The difference in diastolic pressure was no statistically significant between groups(F=1.112, P=0.298), time effect was statistically significant(F=16.794,P<0.001), and there was interaction effect between time and group(F=7.593,P=0.007).(4) The difference in urine volume was statistically significant between groups(F=7.793, P=0.007), time effect was statistically significant(F=87.443, P<0.001),and there was interaction effect between time and group(F=37.505, P<0.001).(5) The difference in ultra-filtration volume was statistically significant between groups(F=4.054, P=0.047), time effect was statistically significant(F=169.967,P<0.001), and there was interaction effect between time and group(F=63.510,P<0.001).(6) The results of generalized estimating equation indicated that there was no statistical significance in treatment effect of edema degree( Wald χ2=2.888, P=0.089),OR was 1.931, there was statistical significance in time effect of edema degree( Waldχ2=53.708, P<0.001), OR was 0.361、0.474、0.758.ConclusionsKin supervision can improve volume knowledge level, self-efficacy, volume management behavior and volume overloaded status of patients with peritoneal dialysis.
