Topical Application Of Tranexamic Acid On The Postoperative Efficacy Analysis Of Hidden Blood Loss For Intertrochanteric Fractures In The Elderly

ObjectiveFor topical application of tranexamic acid intervention of intertrochanteric fractures of femur prevent rotation intramedullary nailing(PFNA)internal fixation of the postoperative efficacy and safety of the recessive loss of blood?MethodsSelect 60 60-80 years old in June 2015-June 2017 hospitalized in our hospital,and smoothly in our hospital surgery patients with intertrochanteric fracture,PFNA intertrochanteric fracture fixation surgery patients with 60 cases were randomly divided into treatment group(n = 30 cases)and control group(n = 30),the treatment group in front of the screw in the tail cap to 0.25 g of tranexamic acid 50 mL injected the bone marrow cavity;Control group shares marrow cavity injection of saline 50 mL.Two groups of 24 h after surgery to give in heparin sodium injection of 0.4 ml subcutaneously,two groups of hidden blood loss calculation,dominant blood loss,perioperative blood loss,postoperative 1,determination of hemoglobin(Hb),RBC 3 days deposited(HCT),recording operation time,length of hospital stay,blood transfusion and the total cost of a hospital,according to the observed two groups of symptoms related complications.ResultsTreatment group and control group in gender,age,body mass index,fracture type and diabetes and no statistically significant difference were observed in the case of hypertension(P > 0.05),treatment group total hidden blood loss(606.58±35.47 ml VS 1133.41±36.39ml)? postoperative bleeding(606.58±35.47 ml VS 1133.41±36.39ml)?blood transfusion(68U VS 32U)? blood transfusion rate(50% VS 30%)? length of hospital stay(8.57±1.591 days VS 11.77±0.675 days)? total cost of a hospital(39916.9±1606.4 yuan VS 49259.3±2197.5 yuan)were less than the control group(P < 0.05).The treatment group 3 days postoperatively Hb(104.4±1.841 g/L VS 91.45±2.225 g/L)?HCT(0.31±0.005% VS 0.27±0.006%)is higher than the control group(P < 0.05),before and after 1 day of Hb? HCT level,operation time difference has no statistical significance(P > 0.05).ConclusionLocal use of tranexamic acid can obviously reduce the closed reduction and PFNA intertrochanteric fracture fixation,intraoperative and postoperative hidden blood loss significantly shorten the hospitalization time,decrease the rate of blood transfusion,reduce the economic burden of family members,and do not increase the risk of thrombotic diseases.
