Objective:To investigate rheumatoid arthritis(RA)patients with total knee replacement used vary dose of tranexamic acid.Then we observed the incidence of lower extremity deep vein thrombosis,pulmonary embolism.At the same time,we also observed the influence of each group were compared in the total blood loss,postoperative 24 h of traffic,reduce postoperative day 3 hemoglobin values and different quantity of blood transfusion,blood transfusion rate.Finally we will determine the safty and efficacy of tranexamic acid in rheumatoid arthritis patients following total keen arthroplasty.Methods:A retrospective study was performed in 60 RA patients following primary unilateral TKA from January 2014 to october 2017.The patients were divided to three groups based on the regime of tranexamic acid: no application of tranexamic acid 30 patients(group A);.21 patients received 1g of tranexamic acid intravenously 10 miniates preoperative combined 1g of tranexamic acid was injected intra-articular before Sutured joint capsule(group B);20 patients received 1g of tranexamic acid intravenously 10 miniates preoperative combined 1g of tranexamic acid was injected intra-articular before Sutured joint capsule and repeatedly intravenous 3 hours postoperativegroup C).Reuult:The basic data(height,weight,gender,BMI)of the preoperative patients were not significantly different,and the preoperative hemoglobin content of patients was not significantly different.No lower extremity venous thrombosis and pulmonary embolism occurred in all 3 groups.In the case of 24 h drainage,total amount of blood transfusion during perioperative period,blood transfusion volume,and the decrease of hemoglobin in the third day after surgery,group B and C were significantly lower than group A,with statistically significant difference(P<0.05).For patients with postoperative blood transfusion rate,group B and group C were significantly lower than group A,with statistical significance(P<0.05).However,the total amount of blood loss of 24 h after operation was lower than that in group B,which was statistically significant(P<0.05).Conclusion:Tranexamic acid for total knee arthroplasty in patients with rheumatoid arthritis in the first time,which does not increase the incidence of lower limb venous thrombosis and pulmonary embolism,and effectively reduce the amount of total perioperative blood loss,blood transfusion and postoperative 24 h flow and reduce blood transfusion rate,also can reduce postoperative hemoglobin falling on the 3 day.Finally increase its dose appropriately can give full play to the functions of the bleeding.
