Objective:To study the effect of HTD on the expression of Notch2,DLL3 and PDGF in rabbits with steroid-induced femoral head necrosis(SANFH)before,during and after modeling,and through light microscopy and electron microscopy The pathological changes of femur bone tissue specimens in rabbits were studied to investigate the efficacy and mechanism of HTD in preventing and treating SANFH.Methods:192 healthy Japanese rabbits were randomly divided into prevention+ treatment group A,treatment group B,model control group C,and blank control group D,48 in each group.All three groups A,B,and C were modeled using Hexijing's method from the third week.The cycle of the model was 6 weeks.Group A started to feed the same amount of Huoxue Tongluo decoction Chinese medicine feed 1 week before the modeling;Group B was fed the Huoxuetongluo decoction Chinese medicine feed at the beginning of the model;C and D groups were fed the same dose of common feed.Dynamic observation of the general conditions of rabbits in each group,and the use of RT-PCR method to determine the gene expression of Notch2,DLL3 in rabbit blood at the end of the second weekend,the fifth weekend,the eighth weekend,using Elisa method to detect rabbit blood The content of PDGF in the.At the same time,rabbit femoral bone specimens from both sides of the rabbit were taken for pathological examination and the ratio of empty lacuna was calculated.Results:1.Notch2: The expression of Notch2 gene in groups A,B,and C increased at the end of the second weekend,the fifth weekend,and the eighth weekend.At the 2nd weekend,Notch2 was higher in group A and B than in group D(P<0.05);Notch2 was higher in group C than in group B(P<0.05);Notch2 was higher in group C than in group D(P<0.01).At the end of the 5th week,Notch2 was higher in group A,B and C than in group D(P<0.01).At the 8th weekend,Notch2 was higher in group B and C than in group D(P<0.01);Notch2 was lower in group B than in group C(P<0.05).2.DLL3: In group A and B,DLL3 showed a decreasing trend at the end of the second weekend,the fifth weekend,and the eighth weekend.DLL3 in group C showed a tendency of increasing at the second weekend,the fifth weekend,and the eighth weekend.At the end of the second weekend,the levels of DLL3 in group A,B and C were significantly higher than those in group D(P<0.01).At the end of the fifth weekend,the DLL3 in group C was higher than that in group D(P>0.05);the DLL3 in group C was higher than A;In group B,group B(P>0.05);at the end of the 8th week,DLL3 in group A was lower than that in group D(P<0.01);DLL3 in group B was lower than that in group D(P<0.05);DLL3 in group C was higher than that in A and B;Group(P>0.05).3.PDGF: From the 2nd weekend to the 8th weekend,PDGF in group A,B and C showed a decreasing trend P>0.05;at the second weekend,PDGF in group C was higher than that in group D(P>0.05);PDGF in group A and B was lower than that in group A and B.Group C(P>0.05).At the end of the 5th week,PDGF in group C was lower than that in group A and B(P>0.05);PDGF in group A,B,C was lower than that in group D(P>0.05).At the 8th weekend,PDGF in group A and B were significantly higher than that in group C(P<0.05);PDGF in group B and C was lower than that in group D(P<0.05).4.In terms of ratio of empty lacun:the ratio of empty lacuna in group C increased significantly;Compared with group C,group B had a slower increase,and group A showed a decreasing trend in the later period.5.Microscopic examination results:The bone trabeculae in group A is thicker and more densely organized.Newly-generated bone cells are seen,and there are more hematopoietic cells in the marrow cavity.In group B,the bone trabeculae was thicker,no obvious fissures were observed,new osteoblasts appeared,the number of hematopoietic cells in the marrow cavity decreased,and there was no large number of hyperplasia in adipocytes.In Group C,the bone trabeculae became thinner,structurally disorganized,partially broken and collapsed,fat cells increased,and hematopoietic cells in the marrow cavity decreased.In group D,the histomorphology of the bone was complete and the structure was clear.Conclusion:1.The occurrence and progression of SANFH were positively correlated with the expression of Notch2,DLL3 and inversely related to the expression of PDFG.Huoxuetongluo decoction can down-regulate the expression of Notch2 and DLL3 gene and the rate of hollow bone lacunae in femoral head,and maintain the concentration of PDGF in blood.It was shown that Huoxue Tongluo Decoction can reduce the effects of hormones on blood vessels,promote the repair and generation of blood vessels,and improve the blood supply of femoral heads.2.The mechanism of prevention and treatment of SANFH by Huoxue Tongluo decoction may be through inhibiting the expression of Notch2,DLL3 gene,upregulating the expression of PDGF to stimulate angiogenesis,accelerating the remodeling of blood vessels,improving the blood supply of femoral head,and stimulating the replication of osteoblasts.Bone formation.To achieve the prevention and treatment of this disease.3.This research topic is based on the traditional Chinese medicine guiding ideology of "No Disease Prevention",changing passively to active,applying Huoxue Tongluo Decoction to intervention treatment before the occurrence of
