| Purpose: To evaluated the DWI-MRI value in estimating the depth of myometrial invasion?cervical stromal invasion and lymph node metastasis.Material and Methods: This is a retrospective study.This study compared clinical and surgical pathology data from 425 patients with endometrial carcinoma treated in our hospital between January 2014 and June 2017.Inclusion criteria included the patients diagnosed with endometrial carcinoma who were operated in our hospital and the patients had complete data.Before the operation the patients had the DWI-MRI and transvaginal color doppler ultrasonography examination.Exclusion criteria included reception of other methods of treatment for endometrial carcinoma including radiotherapy(as primary treatment)and neoadjuvant chemotherapy,patients whose records were incomplete.Endometrial carcinoma patients were staged as: stage?A,n 340;stage?B,n 50;stage ?,n 3;stage ?,n 29;and stage ?,n 3.Results:1 ? Based on the pathological staging after surgery,the 425 endometrial carcinoma patients were staged according to the current FIGO2009 staging as: stage?A,n 340;stage?B,n 50;stage ?,n 3;stage ?,n 29;and stage ?,n 3.The DWI-MRI clinical staging accuracy for endometrial carcinoma?–? by FIGO2009 were 89.11%(303/340),94.00%(47/50),100%(3/3),89.66%(26/29),and 100%(3/3),respectively;the transvaginal color doppler ultrasonography examination clinical staging accuracy for endometrial carcinoma stages ?–? by FIGO2009 were 43.23%(147/340),66.00%(33/50),33.33%(1/3),31.03%(9/29),and 33.33%(1/3),respectively.We have observed that the differences between DWI-MRI examination and the transvaginal color doppler ultrasonography examination,and the differences were statistically significant.2?DWI-MRI examination to the patients without myometrial invasion,the coincidence rate was 76.92%(50/65),the coincidence rate of transvaginal color doppler ultrasonography evaluation was 49.32%(32/56);for 297 cases of patients who were endometrial carcinoma with superficial myometrial invasion,the coincidence rate of DWI-MRI was 90.13%(265/294).The coincidence rate of transvaginal color doppler ultrasonography evaluation was 41.01%(122/294);for patients with endometrial carcinoma with deep myometrial invasion,the diagnostic accuracy of DWI-MRI was 93.94%(62/66).The coincidence rate of transvaginal color doppler ultrasonography examination was 63.64%(42/66).There were 13 cases of endometrial carcinoma with cervical stromal invasion.The diagnostic accuracy of DWI-MRI was 92.30%(12/13).The compliance rate of transvaginal color doppler ultrasonography evaluation was 38.64%(5/13).The differences were statistically significant.Conclusion: DWI-MRI can accurately detect the superficial or deep myometrial invasion,cervical stromal invasion and lymph node metastasis. |
