Objective:To discuss the change of the serum bilirubin level in patients with coronary heart disease and the relationship with severity of coronary artery.Methods:Patients were selected from June 2015 to December 2016 in department of Cardiology,Affiliated Hospital of Qin gdao University Medical College.100 patients with coronary heart disease(CHD),including 50 cases of single coronary artery disease and 50 cases of multivessel coronary artery disease were selected according to the results of coronary angiography.In addition,50 patients without coronary artery disease were selected as control group.Collecting the age,gender,history of high blood pressure and diabetes,blood lipid levels,smoking history,echocardiography,electrocardiogram,general clinical data.All subjects without severe liver and kidney damage,liver disease,cancer cachexia,severe acute infection,blood system diseases,rheumatic disease and diseases affecting the level of serum bilirubin and no use of cholesterol-lowering drugs recently.Detecting and comparing the CHD groups and control group of serum total bilirubin(TBIL),direct bilirubin(DBIL)and indirect bilirubin(IBIL),fasting blood glucose(Glu),triglyceride(TG),total cholesterol(TC),high-density lipoprotein(HDL-c),low density lipoprotein(LDL-c);serum bilirubin level was compared between Single lesion group and multivessel lesions group;The relationship between the serum bilirubin level and the severity of coronary stenosis was analyzed according to the coronary artery Gensini score.Pearson method was used to study the serum bilirubin level and Gensini score linear correlation analysis.Patients with coronary heart disease were followed 3months,6 months and 1 year after treatment.Comparing changes of serum bilirubin level in patients with coronary heart disease before and after treatment.Results:The serum level of TC,TG,and LDL-c,FBG of in CHD group were significantly higher than the control group,whereas serum HDL-c level were lower than the control group,the difference was significant statistically(P < 0.05);The serum level of TBIL,DBIL,IBIL in CHD group were lower than the control group obviously,the differences had statistical significance(P < 0.05);serum TBIL,DBIL,IBIL in multivessel lesions group was significantly lower than that of single lesion group,the difference had statistical significance(P < 0.05),The serum TBIL,DBIL,IBIL level of coronary Gensini score <45 patients was significantly higher than patients withcoronary artery Gensini score?45,the difference had statistical significance(P < 0.05);the study showed that serum bilirubin level and coronary artery Gensini score was presented negative relativity: Total bilirubin was negatively correlated with Gensini score(r =-0.373,P < 0.05),with Direct bilirubin(r =-0.315,P < 0.05),and with Indirect bilirubin(r =-0.296,P < 0.05).Patients with coronary heart disease were followed 3 months,6 months and 1 year after treatment.Drugs were eated roughly the same,including antiplatelet aggregation(aspirin and clopidogrel hydrogen sulfate or Greg los),resistance to hardening of the arteries(statins),beta blockers,nitric acid lipid drug.The serum TBIL,DBIL,IBIL of patients with CHD treated after 3 months were not increased(P > 0.05);while the serum TBIL,DBIL,IBIL of patients with CHD treated after 6 months and 1year were increased(P < 0.05),And the serum TBIL,DBIL,IBIL of patients with CHD after treatment 1 year were obviously higher than 3months of treatment,the difference was statistically significant(P < 0.05).Conclusion :Serum bilirubin levels may be associated with the occurrence of coronary heart disease(CHD)development.The heavier the coronary heart disease lesion severity,the lower the serum bilirubin level.Serum bilirubin levels reduce as severity of coronary artery aggravate.It is of great significance that serum bilirubin level may help diagnose the severity of coronary artery.
