Objective Psoriasis is a common chronic relapsing inflammatory skin disease,of which psoriasis vulgaris is the most common,according to the disease stage,can be divided into three stages of advanced,stationary and remission.The pathogenesis of this disease is still unclear.At present,it is believed that this disease is a polygenic hereditary disease that can be induced by a variety of causes,such as trauma,infection,environment,drugs,mood,living habits,etc.,which may induce or aggravate the disease..At present,more research data indicate that Th7 cells play an important role in the pathogenesis and progression of psoriasis.This article uses a systematic evaluation method to explore the differential expression and significance of Th17 cells in peripheral blood and skin lesions of patients with psoriasis vulgaris in the advanced and stationary phases,in order to expect further understanding of the pathogenesis of psoriasis.To investigate the relationship between Th17 cells and the progression of psoriasis vulgaris,and provide reliable theoretical data for monitoring the progression of psoriasis vulgaris..Methods Computerized search of Wanfang,CNKI,VIP,CBM,Pub Med,Embase database,including all the literature on case-control study on the relationship between psoriasis vulgaris and Th17 cells in advanced and quiescent states,exclusion review,conference papers,Dissertations and case reports and other types of literature;Retrieval time limit set from building to March 2017;Develop strict inclusion and exclusion criteria;A total of three evaluators evaluated the quality of the included studies.Disagreements were resolved through discussions and access to relevant information.In addition,a researcher to complete the research data and data extraction and collation,a researcher to check;According to the different specimens,they were divided into lesion tissue group and peripheral blood group.To further explore the source of heterogeneity,the peripheral blood components were divided into four subgroups according to different observation indexes.They were Th17/CD3+ T cells respectively.Group,Th17/CD4+ T cell group,Th17 cell ratio group,and CD4+IL-17+ T cell ratio group;Finally,Rev Man 5.3 software was used to perform meta-analysis including result analysis,heterogeneity analysis,sensitivity analysis and post-publication risk assessment.Results A total of 12 articles were included,including 12 studies;The results of Systematic review suggest that the expression of Th17 cells in peripheral blood and skinlesions in advanced psoriasis patients is higher than that in quiescent psoriasis patients [SMD = 2.15,95% CI(1.28,3.02)].According to the different nature of the specimens,divided into two subgroups,the results showed that the proportion of Th17 cells in patients with advanced psoriasis lesions was higher than that in patients with quiescent psoriasis,the difference was statistically significant [with lesions as the specimen: SMD combination = 3.77,95% CI(1.92,5.62),P <0.000 01;peripheral blood as specimen: SMD combination = 2.04,95% CI(1.14,2.93),P <0.000 01].In peripheral blood as a specimen of the study,but also to represent the different indicators,divided into four subgroups,the results suggest that patients with psoriasis pituitary progressive high expression of Th17 cells,psoriasis progress and Th17 cells[(SMD pooled = 0.29,95% CI(-0,05,0.63),P = 0.09(Th17 / CD3 + T cells);SMD pooled = 1.53,95% CI(0.88,2.18),P <0.000 01(Th17 / CD4 + T cells);SMD pooled = 2.79,95% CI(1.61,3.97),P <0.000 01(Th17 cell ratio);SMD pooled = 6.16,95% CI(-6.12,18.45),P = 0.33(CD4 + IL-17 + T cells)];Sensitivity analysis results show that the results of this study are stable and reliable;4.Funnels showed publication bias in the literature included in this study.Conclusion The expression level of Th17 cells in the advanced psoriasis group was higher than that in the stationary psoriasis group;The progression of psoriasis has a certain correlation with the expression of Th17 cells.The increase of Th17 cell content may indicate the progression of the disease.
