Evaluation Of The Operation Effect Of Family Physician Services In Chognqing Yuzhong District

Objective: Family physician services were fully implemented in China,in 2016,after it was succeeded in Shanghai,Shenzhen and Beijing.In order to understood the details implement of family physician services in Chongqing Yuzhong,this research investigated the current situation of family physician services in Chongqing Yuzhong district,and summarizes difficulties and experiences about it.And pointed policy suggestions on how to take the construction of family physician services.Method: Consulting literatures of family physician services in database,summarized research status,and used “results chain” framework comprehensive evaluation system in Shanghai family physician services pilot” for our research.Family physicians and patients' questionnaires were compiled according to the evaluation system,and took a test to validity and reliability of questionnaire after pre-survey.We had expert interviews to understand the current situation of implementation in the family physician services through the interviewing method.Using the cluster sampling and random sampling methods for family physicians and community residents respectively.And selecting 60 family physicians and 322 community residents,who came from community health-care centers in Yuzhongdistrict,for questionnaires.The main content of this investigation included basic informations,service needs,supply and usage of family physicians and community residents.The data were inputted Epidata 3.0 and analyzed by SPSS19.0.Using single factor chi-square analysis,and logistic multi-factor analysis to analyze influence factors of willingness of family physicians to change their job and residents' act of signing contracts.Moreover,analyzing service demands of family physicians and community residents according to “output” of suppliers and “results” of demanders and the relation between supply and usage so as to evaluate service effects.Result: Family physician services in Yuzhong district had been implemented,up to 2017,for 3 years.And it had basically established corresponding family physician publicity,talent cultivation and referral mechanism.In addition,Yuzhong district had 11 family physician studios,46 family physician service teams,and proportion of family physicians reached to 23.83% and contracted amount surpassed 130,000.The survey indicated that the old with chronic diseases had a strong willingness to sign a contract;outpatient service made up a big proportion in family physician service,referral service participation and satisfaction of contracted residents are lowest,and family physicians had a lower satisfaction to their salary.Meanwhile,92.19% of contracted residents who participated in the survey had developed a habit of choosing family physician at first.Conclusion: We concluded that Yuzhong District had basically reached the target of family physician service,the way of operation in family physician services had been clarified,the construction of a talent team still needs to be strengthened,the funding support was partial,the propaganda mode was diverse,the referral mechanism had basically beenestablished,and information construction was being gradually implemented.result.There were lack of government funding,insurance support in family physician services.Family physicians had low education level and salaries.Family physician services were lack of government funding and insurance support in government level.And the young had low sign awareness who was middle age.Therefore,to implement the family physician services fully and effectively.The government need to increase investment and set special fund for family physician services,and set health-care subsides for different people.Building the sharing and exchange of integrated medical conjoined structure at last.
