Superficial soft tissue mass is a common disease in clinical.Coughing and shortness of breath can occur when neck mass oppresses trachea.When the mass oppresses nerve,it can cause numbness and stabbing of limbs.It affects daily life of patients.Mass in the face not only affects the beauty,but also brings great mental pressure to patients.The pathological types of superficial soft tissue masses are various,the sources are complex,and the treatment schemes of different pathological types and sources are different,so accurate diagnosis is particularly important for the formulation of the treatment plan.Clinical palpation can detect some superficial soft tissue masses,but it can’t identify the origin and the adjacent condition.It also has difficulty in judging the size of the large masses.Ultrasonic examination can make up the above defects.The common pathological types and sonographic features of superficial soft tissue masses have been summarized in a large number of literatures.The clinical features of superficial soft tissue masses were further analyzed on this article.Objective The common pathological types,sonographic features and clinical features of superficial soft tissue masses were summarized and analyzed in order to improve the correct diagnosis rate of superficial soft tissue masses and provide valuable information for clinical treatment.Methods Review of the PACS system and medical record system to obtain the ultrasonic image data,ultrasonic examination report and medical records of 236 cases(248)of our hospital from January 2015 to July 2017 after penetrating biopsy or postoperative pathology diagnosis.The sonographic features of superficial soft tissue masses of different pathological types,such as quantity,size,boundary,shape,echo and blood flow classification were analyzed.The relationship between superficial soft tissue mass of different pathological types and age,sex,the location of mass and so on was analyzed.Results In this group,248 masses included 25 pathological types,of which 230 were benign,92.7%(230/248),and 18 were malignant,7.3%(18/248).The main pathologic type of benign masses: 87 lipomas,53 epidermal cysts,21 hemangiomas.The main pathological types of malignant masses: 6 malignant peripheral nerve sheath tumors.The main sonographic manifestations of lipomas were spindle or elliptic masses,60 had clear boundary,45 were hypoechoic,52 were visible with a separate piece of light strip that is parallel to the skin,and 80 CDFI had no blood flow signals.The main sonographic manifestations of epidermoid cysts were round or round masses,38 were hypoechoic or anechoic,15 can see dot high echo inside,and 50 CDFI had no blood flow signals.The main sonographic manifestations of hemangiomas: 14 had clear boundaries,14 appeared hypoechoic and mixed echoes,7 showed striate anechoic or calcified,and 18 CDFI had no blood flow signals.The blood flow detection rate of benign masses was 27.8%,and that of malignant masses was 83.3%,the rank and test showed that the difference in blood flow signal classification of benign and malignant masses was very significant(P < 0.01).The average age of lipomas was 51.6 ± 11 years old,of which 71 cases were single and 7 were multiple.The location of masses were trunk 44.8%(39/87),extremities 35.6%(31/87),head and neck 19.5%(17/87).Epidermoid cysts included 46 males,5 females,30 traumatic history,3 of which were accompanied by pain.The analysis of the clinical characteristics of lipomas and hemangiomas showed that there was no significant difference in sex(P>0.05),but the difference of location,age was statistically significant(P<0.05).Conclusions Though superficial soft tissue masses have complex ingredient and many classifications,ultrasonic images of common masses such as lipomas and epidermoid cysts have some characteristics.Such as the lipoma appears as a spindle or oval subcutaneous mass,the internal echo is different,main manifestation is hypoechoic.There’s a separate piece of light strip that is parallel to the skin,generally no color blood flow signal was found.The epidermoid cysts are more common in men,offen had a history of trauma,which can be hypoechoic and anechoic,and can be seen when the machine or haematoma is formed.The echo can be enhanced in the rear,and the CDFI has no blood flow signals.The difference of location,age between lipomas and hemangiomas was statistically significant and patients of hemangiomas were younger than patients of lipomas.When ultrasonography is difficult to identify benign or malignant tumors or pathological types,it can be combined with clinical considerations.CDFI has a certain value for the differentiation of benign and malignant superficial soft tissue masses,and the detection rate of blood flow signal is usually higher in malignant than that of benign mass,and the blood flow signal is more abundant.Therefore,the ultrasound examination of superficial soft tissue masses can provide a reliable diagnostic basis for clinical diagnosis.
