Cases Report Of Malignant Tumors Complicated With Autoimmune Diseases And Review Of The Relative Literatures

Objective:To summarize the clinical characteristics of malignant tumors complicated with autoimmune diseases and to explore the interrelation between them so as to raise awareness of malignant tumors complicated with autoimmune diseases.Methods:The data of 278 patients with malignant tumors complicated with autoimmune diseases was collected in the Fourth Hospital of Hebei Medical University from March 2010 to June 2017 for retrospective analysis.Results:In this study,the top 4 AIDs complicated with malignant tumors were: RA,HT,psoriasis and ITP,with the percentage of 41.1%,20.6%,15.0% and 8.7% respectively.The top 4 malignant tumors complicated with AIDs were: lymphoma,breast,lung and gastric cancer,with the percentage of 27.4%,23.1%,10.7% and 8.2% respectively.RA complicated with the common malignant tumors were lung cancer,lymphoma and breast cancer,with the percentage of 22.6%,20.0% and 20.0% respectively.While HT complicated with the common malignant tumors were lymphoma(48.3%)and breast cancer(28.3%).Psoriasis complicated with the common malignant tumors were breast cancer and gastric cancer,accounting for 22.2% and 20.0% respectively.ITP complicated with the common malignant tumor was lymphoma(56.0%).The lymphomas complicated with the common AIDs were HT(35.4%),RA(28.0%)and ITP(17.1%).While breast cancer and lung cancer complicated with the common AID was RA,accounting for 34.8% and 81.2% respectively in their complicated AIDs.Gastric cancer complicated with the common AID was psoriasis(37.5%).Conclusions:1.There is a dual association between malignancy and AIDs.Patients with autoimmune diseases with poor efficacy for long-term treatment or unexplained clinical symptoms should give priority to the examination of malignant tumors;at the same time,Clinicians should be alert to the possibility of AIDs in the treatment of malignant tumors.2.More than one type of malignant tumors can occur in AIDs,and malignant tumor can be complicated with two or more AIDs.There is a certain link between some malignant tumors,between AIDs,and between malignant tumors and AIDs.3.Malignancy is the main reason of death for patients with autoimmune diseases complicated with malignant tumors,and it is the key factor that affects the prognosis.Anti-tumor therapy should be the first priority and needs multidisciplinary collaborative treatment.The effective anti-tumor therapy may as well be the effective anti-AID treatment.4.AIDs and many malignant tumors have common pathogenesis factors,such as immune disorders and dysbacteriosis,which may be a new way of treatment for the patients in the future.
