ABSTRACTObjective: To investigatethe risk factors and clinical features of Clostridium difficile infection(CDI)in ulcerative colitis(UC).Methods:Clinical features of UC patients with in Department of Gastroenterology,Second Hospital of Hebei Medical University from January 2015 to January 2018 were analyzed retrospectively.Demographic characteristics,clinical features and endoscopic findings of CDI in UC were analyzed.SPSS software was used for statistical analysis.Result:1 A total of 177 UC patients were enrolled in this study,including 90 males and 87 females.40 UC patients(22.6%)were diagnosed with CDI.2 Age,gender,duration of UC,and smoking history had no relationship withCDI in UC patients(P>0.05).3 24 UC patients(60%)in CDI group and 34patients(24.8%)in non-CDI group had previous treatment with glucocorticoids.Meanwhile,5patients(12.5%)in CDI group had administration of infliximab.Treatment with antibiotics,proton pump inhibitors(PPI),immunosuppressants,and 5-aminoinosalicylic acid(5-ASA)had no relationship with CDI(P>0.05).4 There was no difference in ever,abdominal pain,diarrhea,and Bloody stools between two groups(P>0.05).5 Clinical classification,disease activity,and the extent of lesions had no relationship with CDI(P>0.05).6 2 patients(5.4%)in CDI group showed typical pseudomembrane(P=0.009).There was no difference in spontaneous bleeding,deep ulcers,and longitudinal ulcers between two groups(P>0.05).7 Both serum albumin less than 30g/L and hemoglobin less than 90g/L showed no difference between two groups(P>0.05).Conclusion:1 Previous treatment with glucocorticoids and infliximab increased the risk of CDI in UC patients,whereas immunosuppressive agents,antibiotics,5-ASA and PPI didn't influence the incidence of CDI.2 CDI didn't change the clinical manifestations in UC.Pseudomembranous is a specific manifestation of endoscopic for CDI,but the incidence is relatively rare.
