ObjectiveTo investigate the incidence of clostridium difficile infection(CDI)in patients with inflammatory bowel disease(IBD)and the relationship between CDI and clinical characteristics in patients with IBD.MethodsPatients diagnosed with inflammatory bowel disease by electronic gastroenteroscopy and pathological biopsy in the Guangdong Sencond Provincial General Hospital from January 2018 to January 2020 were collected as the IBD group.After exclusion criteria,78 patients were included,including 56 patients with Ulcerative colitis(UC)and 22 patients with Crohn's disease(CD).The control group consisted of 78 patients who had no obvious organic lesions and met the exclusion criteria by electronic gastroenteroscopy in our hospital during the same period.Polymerase chain reaction(PCR)was used to detect clostridium difficile in fecal samples of the two groups.The status of CDI and relevant clinical data of the two groups were collected.General information and clostridium difficile infection were compared between IBD group and control group.Further comparison of the positive rate of CDI in different lesions,disease activity and different severity levels among patients in the IBD group.SPSS 23.0 was used for statistically analysis of the data.When P < 0.05,the results were considered statistically significant.Results1.Comparison of general information between IBD group and control group: There were 78 patients in the IBD group,46 patients in males and 32 patients in females.There were 78 subjects in the control group,of which the male and female patients were 41 and 37.The average age was 40.06±1.67 years old in the IBD group and 41.60±1.92 years old in the control group.There were no significant difference in age and gender between the IBD group and the control group(P > 0.05).2.Comparison of CDI rate: The positive rate of CDI in the IBD group was 11.5%,and the positive rates of CDI in patients with UC and CD were 12.5% and 9.1%,which was significantly higher than that in the control group(0%)and the difference were statistically significant(P<0.05).3.Analysis of the relationship between clinical characteristics and CDI in patients with IBD: The incidence of CDI in patients with active stage and colonic involvement was higher than that in patients with remission stage and non-colonic involvement(P < 0.05).Patients with colonic involvement had a higher risk of clostridium difficile infection than patients without colonic involvement(OR=8.727,95%CI=1.035-73.574,P=0.049).The risk of clostridium difficile infection in mildly active patients was lower than that in non-mildly active patients(moderately or severely active patients),and the results were significantly different(OR=0.098,95%CI=0.011-0.855,P=0.038).ConclusionThe incidence of clostridium difficile infection is higher in patients with inflammatory bowel disease.There is a certain relationship between CDI and clinical characteristics in patients with inflammatory bowel disease.The risk of clostridium difficile infection is higher in patients with colonic involvement and moderate to severe activity stage than in patients with non-colonic involvement and mild activity stage.
