| Purpose: Discuss the possible mechanism of treating allergic contact dermatitis with ershiwuweiercha gel by observing the efficacy of the ershiwuweiercha gel treatment on patients with chronic allergic contact dermatitis and the therapeutic effects on mice with chronic allergic contact dermatitis,in order to provide a theoretical basis for further clinical use.Methods: 1 Clinical observation: 62 patients with chronic allergic contact dermatitis were randomly divided into the treatment group and the control group.The treatment group was given the ershiwuweiercha gel,while the control group was given the hydrocortisone butyrate ointment(You Zhuo Er).Both are for external use,2 times/day,and continuous treatment for 14 days.On the day of the treatment,7 days after the treatment and 14 days of treatment,observe and record the scores of the skin lesion,area,and itchiness.Patients were followed up after a month to check any recurrence.2 Experimental study: 60 Balb/c mice were divided into blank group,model group,positive control group(Hydroxycortisone butyrate group),ershiwuweiercha gel low-dose group,mid-dose group,and high-dose group.Observe the skin lession changes,histopathological findings,thymus and spleen index,and serum levels of IL-17 and TNF-? in mice after the drug intervention to explore the mechanism of ershiwuweiercha gel on chronic allergic contact dermatitis Mechanism.Result 1 clinical observation results: 1.1 After treating for 7 days,the symptoms of the two groups improved.In the treatment group,except for the area of lesions,the scores in the treatment group were lower than the scores before treatment(P<0.05).In the control group,the scores were lower than the scores before treatment other than erythema and the area(P<0.05).1.2 After treating for 14 days,the symptoms of the two groups improved significantly,and the scores were lower than the scores before treatment(P<0.05).There was no statistical difference between the two groups(P>0.05).1.3 After treating for 7 days,for the scale and lichenification,the ershiwuweiercha gel group was more effective than the hydrocortisone butyrate group(P<0.05).1.4 The total effective rates of the ershiwuweiercha gel group and the hydrocortisone butyrate group were 86.67% and 90%,respectively.There was no significant difference between the two groups(P>0.05).1.5 30 days after the treatment,the recurrence rate was 6.67% in the treatment group and 26.67% in the control group.The relapse rate in the ershiwuweiercha gel group was lower than that in the hydrocortisone butyrate cream group(P<0.05).2 Animal Experiment Results: 2.1 Morphological observation of mice skin lesions: Compared with the mice in the model group,the skin lesions of the positive control group and all gel dose groups were improved to various degrees,and the gel mid-dose group improved the most.2.2 Pathological observation: Hematoxylin-Eosin staining microscopy showed the mice had mild epidermal and intracellular edema and inflammatory cell infiltration in the positive control group and all the three ershiwuweiercha gel dose groups.2.3 Measurement of spleen and thymus index: The spleen index of mice in the positive group and the mid-dose group have significant difference with model group(P<0.01,P<0.05).The difference of thymus index in all treated groups with model group was statistically significant(P<0.05).2.4 Determination of IL-17 and TNF-? content: Compared with the model group,the serum levels of TNF-? and IL-17 in the peripheral blood of mice in each gel dose group decreased.For IL-17 reduction,the positive group was superior to the gel dose group(P<0.05);for the TNF-? reduction,the gel mid dose group was equivalent to the positive group,with no statistical difference(P>0.05).Conclusion: 1 The clinical efficacy of ershiwuweiercha gel and hydrocortisone butyrate cream on chronic allergic contact dermatitis are comparable.In treating scale and lichenification,ershiwuweiercha gel is has more immediate relief than hydrocortisone Butyrate Cream.Ershiwuweiercha gel is more conducive to prevent the recurrence of allergic contact dermatitis.2 Ershiwuweiercha gel can effectively improve the skin lesions of mice with chronic allergic contact dermatitis.The mice skin lesions recovered,close to normal skin.It can also reduce the spleen and thymus index of the mice with chronic allergic contact dermatitis,effectively reduce the content of TNF-? and IL-17 in serum,inhibit the activation of T cytokines and the production of inflammatory cells,and interfere the development of allergic contact dermatitis. |
