Clinical Study About Jiawei Shentong Zhuyu Decoction In Treating Axial Symptoms After Open-door Operation Of Cervical Spondylotic Myelopathy

Objective:This study explores the clinical effect by observing the clinical application of Jiawei shentong zhuyu decoction in patients with axial symptoms after posterior unilateral open-door expansive laminoplasty of cervical spondylotic myelopathy,which provide an effective idea and method for prevention or treatment of traditional Chinese medicine.Methods:This study selected 60 patients with cervical spondylotic myelopathy,who has operated unilateral open-door expansive laminoplasty and showed axial symptoms after operation for 1 month,from spine orthopedics of Gansu Provincial Hospital of TCM from September 2016 to October 2017.According to the treatment order,they were randomly divided into treatment group and control group by half and half.Under the circumstances that excluding other situation of neck,and good wound healing without infection and other complications after operation,the treatment group was given Jiawei shentong zhuyu decoction,and control group given oral Celecoxib Capsules.The two groups had the same basic treatment,and actively cooperated with the early cervical functional exercise.In this study,2 courses of treatment and observation were conducted that 2 weeks was a course of treatment.Meanwhile we need to observed monitoring indicators of two groups before and after treatment in different periods(including visual analog pain scale,changes of cervical spine mobility,cervical dysfunction index,cervical JOA scores and axial symptoms).collected clinical data would have statistical analysis by SPSS20.0 statistical software.Then combining with the relevant professional knowledge in this subject,we will give comprehensive evaluation for its clinical efficacy.Results:Through observation and statistical analysis of the treatment group and control group in general information(gender,age,course of the disease),monitoring indicators of two groups before and after treatment in different periods(including visual analog pain scale,cervical spine mobility,cervical dysfunction index and axial symptoms),the results are as follows:1.The gender,age and axial symptoms of the two groups in the treatment group and the control group were statistically analyzed before treatment,and the results showed no significant difference(P>0.05),which had comparable.2.VAS pain score: Comparison of VAS scores in the two groups before treatment had no significant difference(P>0.05)which had comparable;After 2 weeks of treatment,P>0.05,which had no significant statistic difference;After 4 weeks of treatment,P<0.05,that the difference had statistic meaning,which indicated that the treatment group was superior to the control group.3.Improvement of cervical spine mobility: Comparing cervical flexion,lateral flexion and rotation activity in the two groups before treatment,there was no significant difference(P>0.05)which was comparable.After 2 weeks of treatment,the cervical spine mobility(flexion,lateral flexion,and rotation)showed P>0.05 which was not statistically significant,indicating that there was no significant difference in the improvement of cervical spine mobility for two groups in the early stage.After 4 weeks of treatment,cervical spine mobility(flexion,lateral flexion,rotation)showed P<0.05 that the difference was statistically significant,indicating that the improvement in the treatment group was better than the control group.4.Cervical dysfunction index: NDI was compared between two groups before treatment,P>0.05,there had no significant difference that had comparability;The NDI comparison of two groups after 2 weeks and 4 weeks showed P<0.05,and the difference was statistically significant,indicating that the improvement of cervical dysfunction index in the treatment group was better than that of the control group.5.Changes in JOA score of cervical spine: Comparison of JOA scores in the two groups before treatment showed P>0.05,there had no statistically significant difference and was comparable;After 2 weeks and 4 weeks of treatment,P> 0.05 had not statistically significant,indicating that there was no significant difference in the improvement of JOA in the two groups after treatment.6.The improvement of axial symptoms: AS was compared between the two groups before treatment,P>0.05 showed no statistically significant difference and was comparable;After 2weeks and 4 weeks of treatment,P<0.05,the difference was statistically significant,indicating that the improvement of axial symptoms in the treatment group was better than that of the control group.7.The total curative effect of the two groups after treatment: The cure rate of the treatment group was 26.7%,the excellence rate was 43.3%,the efficiency was 16.7%,and the inefficiency was 13.3%.The cure rate of the control group was 6.7%,the excellence rate was30.0%,the efficiency was 43.3%,and the inefficiency was 20.0%.After the rank sum test,P<0.05,the efficacy was significantly different and there was statistical significance,indicating that the total efficacy of axial symptoms in the treatment group was better than that of the control group.Conclusions:Jiawei shentong zhuyu decoction has significant clinical advantages in improving patients with axial symptoms after posterior unilateral open door expansive laminoplasty of cervical spondylotic myelopathy,such as neck shoulder pain,heavy,stiff and acid trapped.It effectively slows the neck pain of patients,improves the function of cervical activity,reduces cervical dysfunction of patients,and eventually improves the axial symptoms.
