Clinical Observation Of Shentong Zhuyu Decoction Combined With ACCF In Treating Cervical Spondylotic Myelopathy

Objective:To observe the clinical effect of Shentong Zhuyu Decoction combined with anterior cervical subtotal corpectomy and fusion(ACCF)in treating cervical spondylotic myelopathy(CSM).Methods:From July 2018 to November 2019,42 patients who had undergone ACCF operation after being admitted to the spine orthopaedics department of Gansu Hospital of traditional Chinese medicine were randomly divided into control group and treatment group according to the sequence of admission and treatment,with 21 cases in each group.The patients in the treatment group took Mecobalamin capsule three times a day,0.5mg a time.The patients in the treatment group took Shentongzhuyu Decoction orally,one dose a day,30 minutes after two meals in the morning and evening.The patients in the two groups received neck function exercise in the early postoperative period.In this study,there were three courses of treatment,one course was 2 weeks,the first course was hospitalization,the second and third courses were follow-up.Observe and record the pain visual simulation score,cervical JOA score and cervical dysfunction index score of all patients in the treatment group and the control group before and after the treatment at the corresponding treatment stage during the hospitalization and follow-up period,collect and sort out the clinical data,and conduct statistical analysis to evaluate the clinical effect.Results:The general information of the two groups of patients in the control group and the treatment group and the visual analog scores of pain,cervical JOA scores,and cervical dysfunction index scores in different treatment periods before and after treatment were statistically analyzed and the following results were obtained:1.The general data of the two groups of patients such as gender,age,and diseased segments were analyzed and compared.The result was P>0.05,with no significant difference.2.Pain visual analogue scale(VAS): Compare the VAS scores of the control group and the treatment group before treatment,P=0.612(P>0.05),no significant difference;the two groups compare P=0.289(P>0.05)after 2 weeks of treatment,The difference was not statistically significant;the comparison between the two groups after 4 weeks of treatment P=0.008(P<0.05)was statistically significant;the comparison between the two groups after 6weeks of treatment P=0.001(P<0.05)was statistically significant.3.JOA score of cervical spine: Compare the JOA scores of the two groups of patients before treatment,P=0.149(P>0.05),no significant difference;the two groups compare P=0.357(P>0.05)after 2 weeks of treatment,the difference is not statistically significant;After 4 weeks of treatment,P=0.661(P>0.05),the difference was not statistically significant;after 6 weeks of treatment,the two groups compared P=0.006(P<0.05),which was statistically significant.4.Cervical spine dysfunction index(NDI): comparing the NDI scores of the two groups of patients before treatment,P=0.487(P>0.05),no significant difference;the two groups compared P=0.103(P>0.05)after 2 weeks of treatment,no difference The significance of the study;the comparison between the two groups after 4 weeks of treatment P=0.036(P<0.05),with statistical significance;the comparison of P=0.001 after 6 weeks of treatment(P<0.05),with statistical significance.Conclusion:Combined with Western Medicine,Shentong Zhuyu Decoction can better improve the changes of postoperative visual analogue score,cervical JOA score,cervical spine dysfunction index,etc.in patients with cervical spondylotic myelopathy;Shentong Zhuyu Decoction can be effective Relieve the symptoms of neck and shoulder soreness,limited mobility,numbness of the upper limbs,and weakness in CSM patients,which is conducive to the early recovery of patients and can improve the quality of life of patients with this disease after surgery.Less response,it is worthy of clinical application.
