Objective: Three-dimensional ultrasound TUI and MRI techniques were used for the prenatal diagnosis of placenta implantation,and postoperative pathological results were used as gold standard.A contrast study with MRI was conducted to explore the diagnostic value of three-dimensional ultrasound TUI technique for placenta implantation and to summarize the different types of placenta implantation.Image features on TUI imaging for early diagnosis and guidance for clinical treatment.Methods: Collected from December 2016 to October 2017 in the Shengjing Hospital Department of Ultrasound prenatal screening of suspected cases of placenta implantation 65 cases,conventional ultrasound examination of the lower edge of the placenta completely covers the cervix within 28 to 36 weeks gestation,are singleton pregnancies and does not exist uterine malformations,and accepting placental MRI examination and production in our hospital.The GE Voluson E8 and E10 three-dimensional ultrasound diagnostic apparatus was used to collect and store the three-dimensional volumetric data of suspected placenta implantation sites.The three-dimensional TUI and HD-Flow techniques and the VCI technique were used to post-process the images.The pathological results as the gold standard,comparing the three-dimensional ultrasound and MRI diagnosis of placenta implantation differences are,and analyze the characteristics of three-dimensional TUI images,the pathological results as the basis for typing,summarizes the different types of placenta implantation ultrasound different types of placenta implantation Image features.Statistical analysis using SPSS 19.0 software.Results: The diagnostic sensitivity of placenta implantation was 81.58%,the specificity was 88.89%,the Youden index was 70.5%,the diagnostic sensitivity of placenta implantation was 74.36%,the specificity was 88.46%,the Youden index was 62.8% Square test,the two test results no significant difference.According to analysis different types of placenta implantation imaging features,the placenta can be divided into three levels of base image: Level 0: The basal part of the placenta echoes tidy,continuous smooth high echo line,regular shape,placental clearance showed clear;Level 1: However,most of the placenta did not break the hypoechoic zone of the posterior placenta,less than 2 incisions could show the disappearance of the posterior placental space,and the appearance of adhesive placenta implantation was consistent with that of the placenta.Level 2: The basal portion of the placenta was irregular in echo,the placenta was thickened,the local morphology was abnormal and changed into a convex shape.The continuous section showed that the echogenic wall of the uterine wall was interrupted,and the implanted type and transperineal placenta implantation were consistent with the type.By placental vascular scan by HD-Flow technique,it was found that the blood vessels perpendicular to the myometrium had higher specificity for diagnosis of placenta implantation.Conclusion: Three-dimensional ultrasound TUI technique has a good diagnostic value for placenta implantation,and its sensitivity and specificity are not statistically different from MRI.By analyzing the characteristics of TUI images of placenta basalis implanted by different types of placenta,placenta implantation was diagnosed according to graded and 0-level propensity.There was a high probability of grafting in grade 1 and grade 2 was more likely to be implanted and penetrated diagnosis.Abnormal vessels that are normal to the myometrium are detected in the placenta and have a high specificity for the diagnosis of placenta implantation.
