Assessment Of Prenatal Diagnosis Of IUGR By Quantitative Three-dimensional Power Doppler Ultrasound

Objectives:To evaluate the differences in placental vascular indices between normal and IUGR pregnancies,providing a quantitative assessment method for clinical diagnosis of IUGR.Methods:A retrospective analysis was conducted on 125 pregnant women whose gestational age was 22 to 39 weeks from January 2016 to December 2017 in the out-patient department of wuxi women and children hospital.Among them,27 cases were diagnosed as IUGR after birth,which was set as IUGR group.Ninety-eight fetuses with normal weight after birth were assigned to the normal group.All pregnant women used three-dimensional power Doppler and VOCAL technique to obtain the placental vascular indices.The two groups were divided into group A(22?27+6 weeks),group B(28?33+6 weeks)and group C(34?39+6 weeks)according to the gestational week.The relationship between gestational age and placental vascular indices was observed through intra-group comparison,and the difference between normal group and IUGR group was observed through inter-group comparison.Results:The values of VI?FI and VFI in the normal group were 16.8±4.68?37.22±4.24 and 6.12±1.43 in group A;19.52±4.08?40.71±3.53 and 7.76±1.93 in group B;18.81 ±3.56?41.37±5.47?8.09±1.90 in group C;respectively.The values of VI?FI and VFI in the IUGR group were 9.41±2.04?32.94±4.11 and 4.22±0.90 in group A;11.77±1.15?36,58± 2.72 and 6.04±1.42 in group B;12.51±1.38?36.02±2.81 and 5.79±0.91 in group C;respectively.Comparison of VI?FI and VFI values of the three groups in the normal group:group B and group C increased compared with group A and the difference was statistically significant(p<0.05).The difference between group B and group C was not statistically significant(p>0.05).Comparison of VI.FI and VFI values of the three groups in the IUGR group:group B and group C increased compared with group A and the difference was statistically significant(p<0.05).The difference between group B and group C was not statistically significant(p>0.05).A comparative analysis of the placental vascular indices revealed statistically significant differences between normal and IUGR pregnancies(p<0.05).The values of VI?FI and VFI in IUGR group were lower than those in normal group.Conclusions:The quantitative assessment of placental vascular indices by means of 3D power Doppler and VOCAL technique has a certain clinical value in differentiating normal pregnancy from IUGR pregnancy.
