| Objective: To observe the clinical effect of taking Huoxue Quyu Decoction to accelerate the dispelling of swelling in the affected limb and reduce the complications of skin and soft tissue for patients with tibial Pilon fracture and explore the mechanism of action of this prescription in Chinese medicine so as to provide basis for application in clinical practice.Methods: A retrospective analysis of 60 patients with closed Pilon fractures who met the inclusion criteria at the Department of Orthopaedics,Affiliated Hospital of Chengdu University of Traditional Chinese Medicine between March 2016 and October 2017 was included.All patients were treated with delayed open reduction and internal fixation,and were divided into control group(n=30)and experimental group(n=30)according to whether or not to take Huoxue Quyu Decoction after admission.After the control group was admitted to the hospital,the Injectio Mannitou was on intravenous drip for detumescence,and the experimental group was treated with Huoxue Quyu Decoction on the basis of the control group.The degree of swelling and pain of the affected limb was evaluated and recorded on the 1st,3rd,5th and 7th day after injury.The time of first appearance of dermatoglyph and the waiting time of surgery was observed and recorded after the injury.The day of admission,the 3rd day and the 7th day after the injury of plasma D-dimer and inflammatory response index was recorded.Data were collected for the swelling rate of affected limbs,VAS score,the time of first appearance of dermatoglyph after surgery,waiting time for surgery,and D-dimer,white blood cell count(WBC)in indicators of inflammation,C reactive protein(CRP)and erythrocyte sedimentation rate(ESR)were analyzed statistically.Results:(1)The general data of the two groups was statistically analyzed.The difference was not statistically significant(P>0.05)and they were comparable.(2)After treatment,the D-Dimer and CRP values of the experimental group were lower than those of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in WBC counts and ESR values between the experimental group and the control group(P>0.05).ESR values of the experimental group and the control group on the 3rd day after injury were compared,P=0.040<0.05.There was a statistical difference between the two groups.The ESR values of the experimental group and the control group were compared on the 7th day after injury,P=0.275>0.05.There was no statistical difference between the two groups.The increase in ESR in the experimental group was lower than that in the control group.(3)Compared with the control group,the swelling rate of the experimental group after treatment was lower than that of the control group,and the difference was statistically significant(P<0.01).(4)After treatment,the VAS score of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.01).(5)The time of first appearance of dermatoglyph and the waiting time for surgery in the experimental group were shorter than those in the control group.The difference was statistically significant(P<0.05).(6)The complication rate of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion: Huoxue Quyu Decoction can effectively relieve the swelling of Pilon fractured limbs and reduce the incidence of skin soft tissue complications,and the efficacy is safe and reliable.It is worthy of further clinical application. |
