BackgroundThe medical risk of hip fracture during perioperative period in elderly patient is common. It is important to reduce the the medical risk of hip fracture during perioperative period in elderly patient. It is important to understand some keys of hip fracture patients during perioperative period, besides for health-care level of hospital and technical level of surgeon. It is common that the elderly patients suffer from hip fracture. It can harm the live of the elderly patients. The mortality of hip fracture is high in the first six month. The mortality of one year after hip fracture is 12~36%. As age goes by, the incidence of hip fracture goes up year after year.ObjectiveTo investigate the complications and mortality of hip fracture in the perioperative period in elderly patient. To clear and definite the risk factors and prevent the complication of the operation, thanks to this investigation.MethodAn retrospective and prospective data of the elderly aged 65 years and over who received treatment between Jan 2007 and Jan 2010 were reviewed. The date comes from The First Affiliated Hospital of Guangzhou Traditional Chinese Medicine University and The Second Affiliated Hospital of Guangzhou Traditional Chinese Medicine University(Guangdong Traditional Chinese Medicine Hospital), which includes the basic information of the patients and the operation, the multiple comparsion of the the complications and mortality, cluster analysis of the Traditional Chinese Medical Syndromes in for the old after operation.All the date should be tested by SPSS 13.0. The standard of the statistics is P<0.05.ResultThe quantity of the hip fracture includes 1015 cases, conservative therapy 157 cases and operation therapy 858 cases.453 cases were femoral neck fracture while 405 cases were intertrochanteric fracture. Six hundred and ninety-three cases (80.77%) had preoperative comorbidities in which the circulation system was most often involved.The average in-hospital time of the conservative therapy was 18.43±15.3 days while the average in-hospital time of the operation therapy was 23.7±12.08 days. Three hundred and seventeen cases(36.95%) had postoperative complications, among which the Pulmonary infection, electrolyte disturbances and urinary tract infection were the most involved. The difference of the postoperative complications among the different age groups has statistical significance.The death toll in operation group is fifteen. Five patients die of the serious Pulmonary infection, four patients die of the pulmonary embolism, four patients die of the acute cardiac failure, one patient die of hemorrhagic shock,one patient die of renal failure. The death toll in conservative group is thirty-four.ConclusionPatients with hip fracture almost suffer from preoperative comorbidities. The mortality of conservative group is higher than the one of operation group. The probability of postoperative complications is high. The positive treatment during perioperative period and elaborate nursing play very impotant part in sugry. The surgeon should perceive the risk factors and tell the advantage and disadvantage. This action could reduce the risk of the operation.
