The Application Of Family Physician And Social Worker Service In Community Hypertension Management

Object:By summarizing the current pattern of community hypertension management,taking the elderly patients with hypertension in the western development zone of Guangzhou City as the research population,to research on the application of family doctors in combination with social worker services in the management of hypertension in the community,to form a standardized,systematic,and new type of community hypertension management process.Methods:This study used a random sampling method to randomly select elderly hypertensive patients in the western part of the development zone,40 people in the home and 40 people in the day care groups.The home group were provided regular outpatient blood pressure control and family doctor team follow-up management.The daycare group were provided regular outpatient blood pressure control,family doctor team follow-up management,day care service from social workers and social volunteers.By observing the blood pressure values of the two groups in the four quarters of 2017,and comparing the body mass index,waist circumference,and blood pressure values before and after the intervention,the effect of family doctors combined with social service management was analyzed.Results:1?There was no significant difference between the two groups in general conditions(gender,family history of hypertension,educational background,annual income level,and medical insurance cost support)(P>0.05).The two groups of patients can be compared.2?After the intervention,the body mass index,waist circumference and systolic blood pressure of the two groups of patients were lower than before(P <0.05),and the diastolic blood pressure was not significantly changed(P>0.05).3?The difference in systolic blood pressure between the two groups was statistically significant during the 4 quarters and diastolic blood pressure difference in the first quarter was statistically significant(P < 0.05),The systolic blood pressure in the four quarters and diastolic blood pressure in the first season of the day care group in 2017,is lower than home endowment group.Conclusion:Through the standardized management of the family doctor team,combined with the services of social workers and social volunteers at day care centers,the body mass index(BMI),waist circumference,and systolic blood pressure of patients can be effectively controlled,and the management of hypertension in the community can be promoted and the management efficiency can be improved.Compared with previous community hypertension management methods,combining social workers and volunteer services can help to save the human resources of the community family doctor team;Social workers and volunteers play a link to help the family doctor team better understand the patient's condition.In this way,not only the shortage of the number of general practitioners in the primary medical institutions can be alleviated,but also the diversified development of the community hypertension management model can be promoted.
