Establishment Of A Training Index System For Postgraduates Majoring In Blood Purification Nursing

ObjectiveTo establish a training index system for postgraduates majoring in blood purification nursing based on a guiding training program for postgraduate students with master degree of nursing in China,and to discuss the conditions how can a master of nursing specialist graduate student who major in blood purification become a clinical nurse specialist.Methods1.Literature review: Research group used “blood purification” “hemopurification” “dialysis” “nurs*” “clinical nurse specialist” “advanced practice nurse” “training” “certification” and so on as the key words to search for the literature related to the subject of our research.2.Semi-structured interviews: In order to make the items of the index system more perfect,we conducted a semi-structured interviews with 15 related personnel,including education experts for master of nursing and doctors/nurses who worked at blood purification room.3.Pre-inquiry by experts: Before Delphi study,we selected 12 experts who are familiar to the research's theme from Qingdao to conduct a pre-consultation.4.Delphi expert inquiry: 30 experts from all over the country were selected to participate in two rounds Delphi study.The experts were required to score the importance of each indicator and sort some indicator at the same level of importance.5.Statistical methods: Described the expert's general data with mean,standard deviation,and composition ratio.The reliability of the study was analyzed by the response rate of the experts,the authority coefficient of the experts and the Kendall's W.The criteria for inclusion of the indicators were: added by experts and agreed by our research group.The exclusion criteria of the indicators were: the mean value of the importance assignment?3.5,the CV?0.2,and the out-of-to-standard ratio is?0.2;or deleted by experts and agreed by our research group.We used the proportional distribution method and percent weight method to calculate the weight.Results1.27 experts from 11 provinces including Heilongjiang,Shanghai,Fujian and so on completed two rounds of consultation.Response rates were 90% and 100% respectively,the authoritative coefficient of experts was 0.78~0.98 which average authoritative coefficient was 0.88.2.The final index system include 4 first-level indicators(training objectives,training process,quality evaluation,certification requirements),19 second-level indicators and 111 third-level indicators.The certification requirements include 6 aspects: academic requirements,work experience,continuing education,research achievement,comprehensive evaluation during the work and unified examination.ConclusionThe enthusiasm and authority of experts are high,and the opinions of experts tend to be consistent.The index system reflects the characteristics of blood purification specialist nursing and can provide some theoretical reference for the training of MNS who will work in blood purification room in China.However,with the development of nursing and the transformation of the roles of CNS in blood purification,the content of the index system established in this study should be adjusted constantly.
