Thyroid hormone can improve cardiac diastolic and systolic function,decrease vascular resistance and increase cardiac output.The genomic and nongenomic actions of T3 on cardiac myocytes and vascular smooth muscle are responsible for significant effects on the heart and cardiovascular system function..Thyroid dysfunction,including subclinical thyroid diseases,can affect heart function,leading to heart failure.Similarly,Paitents with chronic disease states,such as chronic heart failure,often accompanied by low T3 syndrome,mainly manifested as T3 is reduced,reverse triiodothyronine(rT3)is increased,thyroxine(T4)is normal or mild reduced and thyroid stimulating hormone(TSH)is normal.Low T3 syndrome is a independent risk factor for poor prognosis of cardiovascular disease.In patients with heart failure,the decrease in serum T 3 levels is proportional to the severity of heart disease.Heart failure and T3 changes are a vicious circle,low T3 can worsen cardiac function,and heart failure in turn can lead to a decrease in T3 level.Clinical data show that short-term or long-term TH replacement therapy has a good effect in the treatment of heart failure patients with hypothyroidism,subclinical hypothyroidism,low T3 syndrome or simple myocardial infarction.Thyroid hormone is expected to be a new drug in the treatment of heart failure.This review updates research progress on the relationship between thyroid dysfunction and heart failure and the latest evidence for new therapies for heart failure.
