Cervical spondylosis is a common degenerative disease in the elderly population,and it has made serious influence for many people life.In recent years,the incidence of cervical spondylosis are gradually increasing.In all types of cervical spondylosis,cervical spondylosis myelopathy is the most harmful for people.Cervical spondylotic myelopathy is a spinal cord dysfunctional disease caused by degenerative changes of the cervical vetebrates.Currently,the main treatment methods are still operation.The treatment methods of cervical spondylosis myelopathy include anterior cervical decompression and fusion,posterior cervical laminoplasty,posterior cervical decompression and internal fixation,combined anterior and posterior approach.For cervical spondylotic myelopathy with short segmental compression,most doctors use anterior cervical decompression and fusion.However,there are still many controversies about the optimal surgical treatment for patients with multilevel cervical spondylotic myelopathy.Each operation has its own unique advantages and disadvantages.Currently,posterior cervical expansive laminoplasty is widely used for cervical decompression in patients with cervical spondylotic myelopathy involving three or more levels,which has achieved good efficiency in the treatment of multi-segment cervical spondylotic myelopathy.However,postoperative complications,especially C5 nerve root palsy,are still important factors affecting the satisfaction of spine surgeons and patients.Over the last decade,many efforts have been made to understand,foresee and avoid this complication.Unfortunately,the origin of postoperative C5 nerve root palsy is still not clearly understood.The pathogenesis and options for prevention of postoperative C5 palsy remain unidentified and many controversies still exist.We carried out a systematic review of the literature to identify thecharacteristics of disease,risk factors,pathogenesis and other related research,in order to improve the operative efficacy and reduce the incidence of C5 nerve root palsy.
