The Influence Facts Of Recurrent Ischemic Stroke And The Impact Of Statins On The Relapse Of Stroke

Objective: It aims to investigate and analysis the risk factors which may cause the recurrence of ischemic stroke in order to make clinical secondary prevention guidelines.It also tries to understand the compliance of statins in stroke patients for 3 months,6 months and 1 year after discharge.In addition,we also try to find out the impact of statins on stroke recurrence.Method:Collection of patients who suffered from ischemic stroke in Department of Neurology,Second Hospital of Hebei Medical University from October2016 to February 2017.we followed up the patients by telephone for 1 year.Follow-up contents included recurrence,drug taking(especially statins).we compared the relapse rate between the higher-adherence group and lower-adherence group.Single factors analysis used Chisquare,Multi factors analysis used logistic regression.Comparison of recurrence rate between the higher-adherence group and lower-adherence group used Chisquare.The survival curve was drawn by Kaplan-Meier method and the Log-Rank(Mantel-Cox)method was used to determine the effectiveness of statins in the secondary prevention of stroke.Results:1.Altogether 758 cases included in the study.A total of 77 patients relapsed,the relapse rate was 10.15%.The recurrence rate of 3 month,6month,1 year were 3.43%,6.72%,10.15%.2.Results of the logistic regression: sex,diabetes,heart disease,compliance of statins drugs,SAO were the risk factors of ischemic stroke recurrence.3.The rates of taking statins at 3 months,6 months and 1 year were93.13%,81.66% and 72.69%,respectively.The drug compliance of statinswas significantly decreased.4.A total of 32 cases relapsed without taking statins,the relapse rate were 15.45%.45 relapsed in statins group,the relapse rate were 8.16%.There were significant differences statistically in comparison between two groups(X2=5.409,P=0.020).Conclusions:1.Ischemic stroke has a certain recurrence rate,and the cumulative recurrence rate increases gradually with the prolongation of discharge time.2.Male,Diabetes,Heart disease,Compliance of statins drugs,TOAST type of SAO,were the independent risk factors of ischemic stroke recurrence.3.The compliance of statins drugs may affect the recurrence of stroke,and improve the compliance of statins drugs can reduce the recurrence of stroke.Therefor,The propaganda and education about the adherence to the patients should be enhanced in order to reduce the relapse rate of stroke.
