| Polycystic ovary syndrome(PCOS)is a common gynecological disease in puberty and child-bearing age women.With the continuous improvement of living standards,people's eating habits and behaviors become unhealthy.The incidence of PCOS is greatly increased,up to 5%~10%.PCOS is clinically polymorphic,characterized by amenorrhea,anovulatory menstruation,obesity,hairy hair,infertility,ovarian enlargement,high sex hormone level and insulin resistance.Studies have shown that patients with this disease are more likely to suffer from diseases such as diabetes,hypertension,cardiovascular disease,endometrial cancer and so on.Western medicine believes that the disease is mostly associated with genetic,environmental,mental and psychological factors.There is a variety of treatment methods,including the improvement of amenorrhea and other symptoms for ovulatory disorders,high insulin resistance and the final selection of surgical treatment.The clinical uses of drugs are metformin,pioglitazone,Clomifene citrate tablets and other drugs.These drugs often cause some adverse reactions,such as obesity,ovarian hyperstimulation syndrome,luteinized unruptured follicle syndrome,postoperative adhesion etc.In Chinese medicine,kidney deficiency and blood stasis,spleen deficiency and phlegm and liver Qi are considered to be the main causes of this disease.According to the exact etiology and pathogenesis,syndrome differentiation and treatment by acupuncture combined with Western medicine have obvious advantages in the treatment of the clinical symptoms.By combining traditional Chinese with Western medicine,the clinical symptoms such as irregular menstruation and obesity were greatly improved,the level of hormone metabolism was corrected,and the ovulation rate and pregnancy rate were increased.Objective: Using acupuncture and auricular plaster therapy combined with oral metformin to treat obese women with polycystic ovary syndrome through tonifying kidney and Shugan Huoxue,the objective of this study was to observe the clinical curative effect of acupuncture and auricular plaster therapy combined with oral metformin and to improve the acupuncture treatment of polycystic ovary syndrome.Methods: 160 cases of polycystic ovary syndrome were randomly divided into the treatment group and the control group(80 cases in each group)by random digital table.Treatment group: Routine medication of obstetrics and gynecology was not changed.Sterile acupuncture needles(0.30 mm x 40 mm)for single use were inserted at the different acupuncture points after skin disinfection with alcohol cotton.The acupuncture points were selected as the following: the WAN,huaroumen,Qihai,Guan Yuan,Tian shu,Yin Ling Quan,waterways,Sanyinjiao,Xuehai,Zusanli,Fenglong,uterus,wind city and etc,followed by the electrical stimulation for 30 min using electro-acupuncture apparatus.Then patients were prone position,and acupuncture points Pishu,Shenshu,Mingmen,and eight Liao were selected to be strongly stimulated using acupuncture needle.After the needles,thunder-fire moxibustion of Guan Yuan,abdominal uterine points and lumbosacral point eight Ciliao was performed for 15 min.Finally,cowherb seed sticking in a tape was used to press ear acupuncture points uterus,endocrine,spleen,stomach,liver,colon,and hunger point,replacing the ear every 7 days.One course of treatment was one month,the clinical efficacy of acupuncture and auricular plaster therapy combined with oral metformin was evaluated after 6 courses(6 months).The control group was given 0.5 mg/time of metformin enteric-coated tablets,three times a day,one month was a course of treatment,the curative effect was observed after 6-month treatment.After 6 months(6 courses)of treatment,we determined the weight and BMI,hormone level,menstrual cycle,ovulation recovery time,ovarian volume,ovulatory follicle number,the diameter of the largest follicle size,ovulation number,and natural pregnancy rate of two groups of patients.The clinical curative effect of two groups of patients was analyzed and evaluated.Results:(1)The weight and BMI value of the treatment group were significantly lower than those of the control group after treatment(P<0.01),indicating that body weight-reducing effect by combined use of acupuncture and oral metformin was better than oral metformin alone.(2)Compared with control group,the levels of serum sex hormones,including FSH,LH,T,E2,and LH/FSH,were significantly improved in the treatment group(P<0.01).(3)After 2 months,4 months and 6 months of treatment,number of patients who restored the menstrual cycle phase was significantly higher in treatment group than in control group(P<0.01).(4)Compared with control group,the ovulation size,the number of follicles and the largest follicle diameter of the treatment group were significantly improved(P<0.01).(5)Ovulation rate(P<0.05)of the treatment group were markedly increased compared with the control group.(6)The total effective rate in the treatment group and control group was 81.25% and 43.75%,respectively.The difference between the two groups was statistically significant.Conclusion: The total effective rate of acupuncture and auricular plaster therapy combined with oral metformin on PCOS treatment is higher than that of oral metformin alone. |
