Part 1 Clinical characteristics of elderly patients with malignant skin tumorsObjective: By collating the clinical and pathological data of elderly skin tumor patients admitted to the Fourth Hospital of Hebei Medical University from January 2015 to January 2018,the clinical features and epidemiological characteristics of malignant tumors in elderly patients with skin tumors were statistically analyzed.Methods: We selected 554 skin tumor patients aged 60 years or older from the Fourth Hospital of Hebei Medical University from January 2015 to January 2018 to study and analyze 130 patients with malignant tumors.SPSS 21.0 software package was used for data processing and two independent samples of t-test were used.P<0.05 was considered statistically significant.Results: In 554 cases of skin tumors,the proportion of malignant tumors in the upper limb tumors was the lowest,and the proportion of skin exposed areas was the highest.The top four malignant skin tumors are BCC,SCC,Bowen disease,Paget disease.There were 73 cases of head and neck,32 cases of trunk,and 6 cases of perineum,of which 79 cases were in the exposed area(head and neck,hands).Histopathology was dominated by BCC and SCC.The perineum was dominated by extramammary Paget disease,and malignant melanoma had one case in each hand and foot.Conclusions: From January 2015 to January 2018,the proportion of malignant tumors has increased year by year.This shows that with increasing age,the likelihood of suffering from malignant skin malignancy is greater.Head and neck were absolute exposure areas,and malignant tumors accounted for 26.1%.The incidence of BCC and SCC may be related to sun exposure.Ultraviolet radiation is considered to be the most important risk factor.The most common disease in the trunk is Bowen disease,followed by breast Paget disease.Bowen disease may be related to HPV infection,exposure to chemicals such as arsenic.Part 2 Elderly skin malignant tumors and peripheral blood B lymphocytes,NK cell phaseObjective: To analyze the correlation between B lymphocytes,NK cells and tumors in peripheral blood of 30 elderly patients with malignant skin cancer,and to analyze the differences in peripheral blood of the elderly B lymphocytes and NK cells in patients with skin malignancies and non-cutaneous malignant tumors.Methods: Among 130 patients with cutaneous malignancy,30 were selected as experimental groups.Select age of more than 60 years old,outpatient diagnosis of non-skin malignancy 30 patients as a control group.The differences in the levels of B cells and NK cells in the peripheral blood of the experimental and control groups were compared.SPSS 21.0 software package was used for data processing and two independent samples of t-test were used.P<0.05 was considered statistically significant.Results: The mean percentage of B lymphocytes in 30 patients with malignant skin tumors was not statistically different from that in the control group(P>0.05).The mean B lymphocyte count in the experimental group was significantly higher than that in the control group(P<0.01).The NK cell counts and percentages of the experimental group were significantly higher than those of the control group(P <0.05).Conclusions: In 30 patients with cutaneous malignancy,the mean B lymphocyte count in the experimental group was significantly higher than that in the control group.The diametrically opposed conclusions reported by the different study groups in the literature may be due to the positive and negative regulatory effects of B cells and tumor immunity.The NK cell counts and percentages in the peripheral blood of the experimental group were significantly higher than those in the control group,which may be related to the decline of immune function in the elderly.
