Objective: Differential diffusion imaging(Diffusion tensor imaging DTI)was used to determine the difference in fractional anisotropy scores of LA patients group of fall risk,LA patients without fall risk group,healthy control group The comparisons reflect the differences in the structural integrity of white matter fibers in order to study the characteristics of the changes in the white matter fiber bundle microstructure in LA patients,and to explore the correlation between LA and falls,and provide a more abundant diagnostic basis for early prevention of falls in LA patients..Objects and methods:Among the 66 middle-aged and elderly patients(29/35 male/female),22 were in the healthy control group(average age was 66.24±5.84 years),23 patients in the LA group without fall risk(mean age was 67.21±6.11 years)and the risk of LA falls Group 21 patients(mean age 67.38±7.40 years).Whole brain assessments were performed using conventional MRI and diffusion tensor imaging(DTI)sequences.Using the “fiber bundle tracking spatial statistical analysis(TBSS)method” in FSL software,predictors including all features were used as covariates in TBSS.Regression analysis identified and extracted LA patients with falling risk group and LA without falling risk group,LA patients with falling risk group and healthy control group,compared with those with significantly lower FA values,and with LA patients falling risk group MMSE,Mo CA Balance function to do relevant analysis.Result: In the LA fall risk group,the fraction anisotropy FA of the corpus callosum,bilateral corticospinal tract,left internal capsule forelimb,left anterior thalamus radiation,bilateral lower occipital frontal bundles,and left upper longitudinal bundles decreased,compared with LA group without fall risk and the healthy control group.There was a significant difference in FA values between patients without falling risk and healthy controls(P<0.05).In the repeated study,the decrease in FA in the fall risk group of LA patients was not significantly related to the basic data such as age,gender,blood pressure,and educational years of the covariates(P>0.05),but it was related to cognitive function,MMSE,and balance.Functional and relevant(P<0.05).Conclusion:In patients with LA,the integrity of the white matter fiber bundles is gradually destroyed with the development of lesions,and the apparently normal-performing brain white matter microstructure also changes,and the corresponding brain regions are involved,and the TBSS is extracted and extracted from the corpus callosum under the knees and bilaterally.Eight fiber bundles related to movement and balance,such as occipitofrontal tract,bilateral corticospinal tract,left upper longitudinal bundle,left internal capsule forelimb,and left anterior thalamus radiation.Therefore,quantitative analysis of FA values and MD values of DTI parameters in LA patients can be preceded by conventional imaging,and the changes in the microstructure of white matter fiber bundles that cause lower limb dyskinesia and increased risk of fall in patients with early fall risk of LA are found to be at risk of falling.The identification and judgment of illness in LA patients provide important imaging evidence.
