Objective: The purpose of this study was to demonstrate the relevance of H-FABP and GPBB to myocardial injury in sepsis.To explore the effect of H-FABP and GPBB on the severity of disease and clinical prognosis.Methods : A total of 40 cases of children with sepsis were selected in this study from the January 1,2017 to October 31,2017.According to the illness severity was divided into sepsis group(15 cases),severe sepsis group(13 cases)and septic shock(12cases),19 cases of children with non-infectious diseases were selected as control group for that same period,The serum leukocyte,CRP,PCT,c Tn I,CK,CK-MB,CKMB isoenzyme quality and NT-pro BNP were collected within 24 hours of admission.The APECHE II scores were calculated for each child.Using ELISA method to determine the level of H-FABP and GPBB in peripheral blood collected 24 hours after admission,analyze clinical index differences between groups.According to the survival of patients with sepsis,the patients were divided into better group(37cases)and death group(3 cases).All children in the group were improved by ultrasonography and recorded LVEF value,and all children with sepsis were divided into normal LVEF group(26 cases)with LVEF descending group(14cases).According to serum troponin I level,all children with sepsis were divided into the normal c Tn I group(29 cases)and the c Tn I group(11 cases).The differences of FABP3 and GPBB in the groups were analyzed.Based on the decrease of LVEF,using the area of the ROC curve under the ROC curve to measure the value of various indicators in predicting the severity of myocardial injury in sepsis.Results: There were significant differences in FABP3 levels between the control group,sepsis group,severe sepsis group and septic shock group(H= 42.241,P <0.05),and the GPBB level was also significantly different between the groups(H=32.486,P<0.05).Although there was no significant difference between the recovery group and the death group,there was an elevated trend of FABP3 and GPBB in the death group.It was found that FABP3 was statistically significant(t =-3.770,P<0.05)and that that level of GPBB was the same for both group(z =-2.113,P <0.05)in comparison with the level of the new myocardial marker of the normal LVEF group and LVEF decrease(LVEF?60%).There was no significant difference in the level of new myocardial markers between the two groups of c Tn I normal group and c Tn I group,but there was an elevated trend of H-FABP and GPBB in the c Tn I group.The areas under the ROC curve were FABP3(0.821),NT-Pro BNP(0.738),GPBB(0.661),CK(0.560),CTn I(0.512).Conclusion:The serum FABP3 and the GPBB levels can be used to monitor for Sepsis-related myocardial injury,and it has a correlation with the severity of the disease and the prognosis.H-FABP has a significant advantage over traditional myocardial markers in sensitivity and specificity to determine the myocardial injury of sepsis.
