Objective: To study the serum level of heart-type fatty acid- binding protein (h-FABP)in children with Kawasaki disease(KD) and assess the value of h-FABP.Methods: The serum levels of h-FABP,Cardiac troponinⅠ(cTnⅠ) and creatine kinase-MB(CK-MB) were determined in the children with KD(n=48) at the acut and recoverystage and in the control group (n=30) respectively by enzyme-linked immnoabsorbentassay. The internal diameter of coronary artery and the ejection fraction(LVEF) of KDgroup were measured by two-dimensional echocardiography at the acut and recoverystage.Results: (1)The serum level of h-FABP and cTnⅠin KD group was significantly higherthan that in the control group(p<0.01 and 0.05 respectively). there was no significantdifference in serum CK-MB concentrations between the two groups(p>0.05). (2)Thepositive rate of h-FABP and cTnⅠin KD group were higher than that in the controlgroup(p<0.01, 0.05),the level of CK-MB concentration had no significant differencebetween the two groups(p>0.05), and the difference of the positive rate between h-FABPand cTnⅠwas markedly(p<0.01). (3)The difference of the h-FABP concentration betweenCAL and NCAL groups was significant(p<0.01). (4) The h-FABP concentration in KDgroup at the acut was higher than that at the recovery stage(p<0.01), while there were nosignificant change of cTnⅠand CK-MB concentration between the twostages(p>0.05both). (5) The h-FABP concentration in KD group had negative correlationto LVEF (r=-0.35, p<0.05).Conclusion The serum concentration of h-FABP and cTnⅠcan be regared as thebiomarker of myocardial damage in the children with KD, and the fronter has moresensitivity and specificity; the serum level of h-FABP is closely related to the severity ofmyocardial damage in the children with KD. The serurn h-FABP concentration is aguideline of early diagnosis, clinical treatment and prognosis in the children with KD.
