Objectives By the comparison of curative effects with conservative treatment,arthroscopic surgery alone and ultrashort wave after arthroscopy in the treatment of patellofemoral arthritis(Outerbridge classification grade ?,?),focus on early treatment of the disease,slow development.To explorer the curative effects of different treatment methods for patellofemoral arthritis(Outerbridge classification grade ?,?),and provide more choices and evidence for the treatment of this disease.Methods From January 2015 to January 2017,130 cases of patellofemoral arthritis who were hospitalized in Tangshan Union Hospital were selected.Selection criteria:(1)Age 40-60.(2)There are repeated pain in front of the knee.When up and down stairs and leg squats,the symptoms get worse with Peripatellar tenderness.(3)Patellar grinding test positive.(4)X-ray showed no varus or valgus of knee joint and normal patellofemoral joint space.(5)MRI accords with Recht classification standard of articular cartilage,grade I and grade II.(6)The operative group and the postoperative physiotherapy group accord with the Outerbridge grade I and II under arthroscopy.Exclusion standard:(1)Other causes of infection or non-infectious arthritis.(2)Knee joint force line change.(3)Congenital dysplasia of patellofemoral joint.(4)Injury of anterior cruciate ligaments and medial and lateral ligaments of knee joint.(5)Recent use of hormone therapy or intraarticular injection therapy.(6)Suffering from severe heart,lung,brain and kidney diseases.All of the patients divided into 4 groups by random number table method,control group(30 cases),physiotherapy group(31 cases),operation group(31 cases)and postoperative physiotherapy group(30 cases).Each group was treated differently.The control group was given functional exercise for 1 week;Physiotherapy group was given ultrashort wave therapy for 1 week;Operation group was given arthroscopic surgery;Postoperative physiotherapy group was given ultrashort wave after arthroscopic surgery.Evaluation of Joint function used Kujala patellofemoral Joint score and Lequene-Mery scoring system at before treatment,recorded separately at one week after treatment,three weeks after treatment and six months after treatment.The measurement data are expressed as mean ±standard deviation(x ±s).One-Way ANOVA was used to compare the measurement data in each group's baseline value.The comparison of the counting data was conducted by the ChiSquare test.Comparison of intra-group and inter-group joint function scores used Repeated Measures ANOVA.The difference is statistically difference in terms of P<0.05.Results 1 In the control group,there was a statistical difference in the score of joint function between one week after treatment and before treatment,but there was no statistical difference between three weeks after treatment,six months after treatment and before treatment.2 In the physiotherapy group,there was a statistical difference in the score of joint function between one week,three week after treatment and before treatment,but there was no statistical difference between six months after treatment and before treatment.3 There were statistical differences in the score of joint function between one week,three weeks and six months after treatment in the operation group and before treatment.4 There were statistical differences in the score of joint function between one week,three weeks and six months after treatment in the physiotherapy group and before treatment.5 There was no statistical difference in the score of joint function between the control group and the physiotherapy group one week after treatment,but the physiotherapy group was superior to the control group in three weeks after treatment,and the difference was statistical.There was no statistical difference between the two groups six months after treatment.6 There was no statistical difference in the score of joint function between the physiotherapy group and the operative group one week after treatment.The operation group was superior to the physiotherapy group in 3 weeks and 6 months after treatment,and the difference was statistical.7 The Postoperative physiotherapy group was superior to the operative group in 3 weeks and 6 months after treatment,and the difference was statistical.8 There was no statistical difference in the total therapeutic effect between the control group and the physiotherapy group.There was statistical difference between the physiotherapy group and the operation group.There was statistical difference between the operation group and the postoperative physiotherapy group.Conclusions Ultrashort wave therapy after arthroscopy can relieve the clinical symptoms,improve the joint function of patellofemoral arthritis.The effect is better than that of functional exercise alone,ultrashort wave and arthroscopic surgery alone.Ultrashort wave therapy after arthroscopy is recommended in the treatment of patellofemoral arthritis(Outerbridge classification grade ?,?).
