BackgroundGestational diabetes is one of the most common complications of pregnancy.It is defined as: pre-pregnancy diabetes with no diabetes and glucose tolerance,and diabetes during pregnancy or first discovery.GDM happen in the near future will affect maternal and fetal health,forward also increase mother postpartum and the incidence of type 2 diabetes,at the same time can increase the children a variety of metabolic diseases,the incidence of adult,so the effective control of blood glucose is very important.At present,the diagnostic methods and standards for gestational diabetes are not yet fully unified,and the reported incidence rate varies widely,from 1% to 14%.Gestational diabetes risk factors including race,older women(defined as more than 35 years old),polycystic ovary syndrome,hypertension,family history of diabetes and pregnancy overweight and obesity,such as low intensity physical activity such as sedentary lifestyle.Current causes include: genetic factors,insulin resistance(IR),inflammatory factors,fat factors,etc.In addition,an unhealthy lifestyle during pregnancy can also lead to gestational diabetes.The main purpose of GDM management is to optimize blood glucose control and improve pregnancy outcomes.In general,the initial GDM management includes diet guidance for education and exercise.Without adequate blood sugar control,women generally take antidiabetic drugs that directly lower their blood sugar levels,thereby indirectly reducing the baby's blood sugar.GDM affects maternal health,including recent and long-term effects.Diet therapy is the first line of defense for women in GDM.Therefore,for patients diagnosed with A1 stage GDM,nutritional diet intervention can be given first.Research purposeThrough case comparison and analysis,the effect of "one-day outpatient service" on maternal and child pregnancy outcomes in patients with GDM with abnormal glucose tolerance was studied The research methods(1)selection in February 2017-May 2017 set up in shaanxi province people's hospital of obstetrics perinatal health care card,and conform to the inclusion criteria of GDM pregnant women 60 cases as the research object,will in February 2017-August 2017 in 200 cases of pregnant women as control group,and control group according to the inclusion criteria each divided into 100 cases.(2)groups of age,educational background,abortion times,height,weight,before childbirth,the number of new disease,pregnancy,the pregnancy weight gain count,weight after 24 weeks of jin jin,write a week recipe,use of insulin treatment during pregnancy,fasting blood glucose on admission,hemoglobin values on admission,glycated hemoglobin values on admission,weeks termination of pregnancy,childbirth way,with or without premature rupture of membranes,blood sugar,neonatal birth weight and maternal postpartum 42 days OGTT is normal.SPSS20.0 statistical software was used for data processing.The results of the study(1)respectively,for group A,group B and group C in three groups of patients age,family history,history of abortion,fetal single factor variance analysis,the results showed no statistical difference(P > 0.05).The comparable.(2)respectively,for group A,group B and group C in the number of three groups of patients,weight gain after 24 weeks of single factor analysis of variance,the use of insulin,and the results showed no statistical difference(P < 0.05).Statistically significant.(3)statistics were made on new complications of pregnancy in each group,and 7 new diseases were developed.(gestational hypertension,excessive water,premature rupture of membranes,urinary tract infection,fetal malformation,ICP,anemia).Group A had thelowest incidence.(4)the grouping mother pregnancy outcomes related factors analysis of three groups of fasting glucose,glycosylated hemoglobin,after admission hemoglobin among related factors analysis this data,group A success rate higher than the other two groups.(5)by analyzing the outcome of the three groups of mothers and newborns,the difference analysis was conducted between the delivery mode,normal gestational age and neonatal hypoglycemia,and P < 0.05 was of statistical significance.The research conclusion1.The management success of gestational diabetes mellitus is related to patient compliance and is positively correlated;2.Most patients with high compliance have higher education background;3."1" to participate in,can reduce the incidence rate of patients with pregnancy complications(gestational hypertension disease,anemia,sheep overhydration,premature rupture of membranes,urinary tract infection,fetal malformation rate);4.The participation of "one day clinic" significantly improved the adverse outcomes of maternal pregnancy(premature birth,expired birth,large children,hypoglycemia of newborn and delivery mode);5.Education mode of "one-day clinic" is superior to the oral education result of outpatient service,and it is significantly effective for moderate GDM,which can be promoted and implemented clinically.
