| Objectives Based on the Delphi method to construct the clinical nurses prevention MDRPI of critically ill patients for the knowledge,attitude,practice assessment scale to provide the basis for effectively reducing the MDRPI.Methods 1 On the basis of consulting the literature,referring to the latest edition of the NPUAP on Prevention and Treatment of Pressure Ulcers: Quick Reference Guide and consulting the experts,fully analyzing the risk factors of the MDRPI,based on the theory of knowledge,attitude,practice as a conceptual framework,a preliminary questionnaire was prepared.The Delphi method was used to conduct two rounds of consultation with 35 experts in three fields with relation to wound colostomy pressure injury specialist,critical care and nursing management,and these experts come from 20 top three hospitals in 8 provinces and cities nationwide,contributing to finally establishment in respect of the knowledge,attitude,practice assessment scale for prevention MDRPI in clinical nurses information table.2 Totally 204 nurses were investigated in the intensive care unit of the general hospital in Tangshan to detect the reliability,validity and acceptability of the scale.The reliability and validity of the scale were evaluated by content validity,structure validity,internal consistency reliability,split-half reliability,and test-retest reliability.Results 1 Based on the theory of knowledge,attitude,practice,the preliminary item pool was consisted of 3 dimensions and 41 items.The recovery rate of the questionnaire in two rounds of consultation was both 100%;the overall authoritative coefficient was 0.89 and 0.91,respectively,both were more than 0.8;the coordination coefficient was 0.115 and 0.246,respectively,the difference was statistically significant(P<0.05).The second round of expert advice on the importance of each item scores scored 4.03 to 4.89(full score was 5 points),the consent rate was 94.3% to 100%,the coefficient of variation was 6.61% to 21.51%.After two rounds of expert consultations,deleted one item in knowledge dimension and revised the other 10 items(including 3 items of knowledge dimension,1 item of attitude dimension,and 6 items of behavior dimension),the scale constructed by the Delphi method includes 40 items in 3 dimensions(knowledge,attitude,practice).2 Reliability,validity,and acceptability testing of scales constructed by the Delphi method,the scale was consisted of 3 dimensions and 38 items,it can evaluate the knowledge,attitude and practice of clinical nurses in the prevention of MDRPI.The total score of the scale was 38~190 points,the higher the score,the higher the level of the clinical nurses prevention MDRPI of critically ill patients for the knowledge,attitude,practice.The Cronbach's alpha coefficient of the total scale was 0.962,the three-dimensional Cronbach's alpha coefficient was between 0.923 and 0.954,all >0.9;the spilt-half reliability coefficient of the total scale was 0.982,the three-dimensional spilt-half reliability coefficient was between 0.900 and 0.975,all >0.8;the test-retest reliability coefficient of the total scale was 0.971,the three-dimensional test-retest reliability coefficient was between 0.932 and 0.967,all >0.9;The scale-level content validity index was 0.964,as well as the cumulative contribution rate of structure validity was 64.927%.The survey completion time was between 5 and 10 minutes.Conclusions 1 The knowledge,attitude,practice assessment scale for prevention MDRPI in clinical nurses was of high reliability and practicability,which can provide the basis for standardizing nursing behaviors of nurses and effectively reducing the MDRPI.2 The scale has good reliability and validity,Short measurement time and scale items were easy to understand.It can be used as a measure of clinical nurses prevention MDRPI of critically ill patients for the knowledge,attitude,practice level of assessment tools. |
