ObjectiveIn this study,the social support scale of xiao shuiyuan was used to observe the quality of life among the groups.To describe the relationship between different levels of social support and the quality of life of patients with heart failure.And if social support changes that the quality of life of patients with heart failure will be changes.Than by increasing the degree of social support to improve the quality of life,reduce the recurrence and improve treatment compliance,reduce hospitalization rates to improve prognosis,provide new theory and ideas for treatment of heart failure.On the one hand,it can effectively promote the continuous improvement of medical quality.On the other hand,we should promote rational application of medical resources and reduce waste of medical resources.Method1.In March 2017 between January 2018 and in the first affiliated hospital of kunming medical university and willing to participate in the follow-up survey of patients with HF the research object,for the patient and family to informed consent and signed written informed consent began in the study,patients with established personal information archives.2.Patients with baseline information collection,including gender,age,socioeconomic status,education level,work status,marriage status,living status,family income,health care type),past medical history,positive signs,etiology,scale/hf questionnaire(KCCQ,EQ-5D),6 minutes walk test,ultrasonic cardiogram,BNP,clinical incidence of adverse events(hospitalized again,new atrial fibrillation,new renal insufficiency,myocardial infarction,stroke,recovery after cardiac arrest,death).3.The score group was scored with SSRS after discharge:less than 22 divided into lower social support group,23-44 divided into moderate social support group,and 45-65 divided into high social support group.4.Discharged 1 months follow-up,with SSRS repeat the measurement of social support level,according to the change of the level of social support,patients can be divided into five subgroups:score is still less than 22 points for continuous low social support group,the score is still 23-44 divided into sustained moderate social support group,45-65 divided into continuous high social support group,The score reduction group included:the score decreased from 23-44 to 22,and the score decreased from 45-65 to 23-44,and the score decreased from 45-65 to 22.The score increased from 22 to 23-44,from 22 to 45-65,and from 23-44 to 45-65.5.During one months telephone follow-up,questionnaires were collected and the incidence of clinical adverse events(rehospitalization,new atrial fibrillation,new renal insufficiency,myocardial infarction,stroke,cardiac arrest,and death)were collected.6.Use SPSS20.0 statistical software to analyze data,and compare the baseline of high,middle and low social support group in EQ-5D and KCCQ life quality score,6 minutes walk test and NYHA classification have differences,as well as the above five subgroups of patients in the hospital a month later,the EQ-5D and KCCQ life quality score,adherence and difference between the hospitalization;Application of Pearson correlation analysis of social support relation with hf patients quality of life,and build the multiple linear regression model,the analysis of heart failure patients quality of life and the correlation of the variables,with p<0.05 for the difference was statistically significant.Results1.A total of 135 cases were included in the study,three were exited,one died during hospital stay,two cases were found outside the hospital during follow-up,and 129 cases were completed.According to the social support scores measured by the patients when they were discharged from the hospital,there were 61 people in the high social support group,57 in the middle social support group and 11 in the low social support group.Out of the hospital a month to social support score,according to the score and get five subgroups,respectively:continuous high social support group of 56 people,continue to social support group of 46 people,continuous low social support group of 5 people,higher social support group of 12 people,social support lower group of 10 people.2.Patients' gender,educational level,marital status,family annual total income,weekly exercise duration was statistically difference among three groups,such as a higher percentage of men in high social support group,and higher educational level,married,household income is high,the proportion of patients with long weekly exercise is higher,the difference was statistically significant(p<0.05).3.EQ-5D among three groups of patients admitted to hospital the following day there was no statistically significant difference comparing the quality of life score,quality of life score at discharge-admission EQ-5D grade difference and KCCQ score comparison,the result is highly social support score>moderate social support group>low social support group.1 months follow-up EQ-5D score between the 5 groups,KCCQ score comparison,results for low quality of life of the worst group,group with high quality of life is best,higher quality of life is relatively low group,lower life quality is high group difference,p<0.05,the difference was statistically significant.4.The five intergroupe of patients rehospitalization and drug compliance were compared,and the results showed that the higher the social support,the less the number of patients and the better the drug compliance,and the difference was statistically significant.5.Social support score difference(one month after discharge,discharge)and EQ-5D score difference(one month after discharge)was positively related to relationship,social support score increases,the quality of life score also increased accordingly.6.Multiple linear regression model shows that social support score,6 minutes walk test,weekly exercise time and quality of life scores were positively related,and admission,NYHA classification,BNP value and negatively correlated with social support score.Conclusion1.The quality of life in patients with heart failure was significantly correlated with social support.Social support also has a positive impact on the quality of life in a short period of time.The higher the social support,the better the patient's quality of life in the short term.The higher the social support of heart failure patients,the better the drug compliance and the less frequency of hospitalization.2.The improvement or decrease of social support will affect the quality of life.Patients' social support improves their quality of life as well.
