Objective:miRNA-499 is obviously expressed in the blood circulation during myocardial ischemic injury. However, whether miRNA-499 can be a new potential biological marker for clinical diagnosis of coronary heart disease is still divided. This article intends to investigate the differential expression of miRNA-499 in the circulation of acute coronary syndrome and healthy controls, and to compare the expression of iRNA-499 in patients with acute myocardial infarction and angina pectoris,as well as to analyze the clinical significance of different miRNA-499 expressions in the diagnosis of acute coronary syndrome.Method:A total of 106 cases received coronary angiography examination in the Department of Cardiology of the Second Affiliated Hospital of Nanhua University from May 2017 to October 2017 are included.According to the examination results and diagnostic criteria, patients were divided into acute coronary syndrome group and the control group. The acute coronary syndrome component was 28 cases of acute myocardial infarction(AMI)patients and 47 cases of unstable angina(UA)patients.31 cases of non-CHD healthy population were the control group.Subjects in the AMI group received peripheral blood within 12 hours of symptom onset, and non-coronary heart disease healthy subjects were collected fasting blood samples on the following morning. The total mi RNA content in peripheral blood of all enrolled populations was extracted, and the mi RNA-499 content was detected using SYBR qrt-PCR.Basic data, past medical history, tobacco and alcohol history, family history, etc.and blood biochemical indicators of the selected population are collected.Integrative analysis of clinical indicators and plasma mi RNA-499 expression in different correlation.Result:(1)There were no significant differences in the basic clinical data, such as the ratio of male to female, age, history of drinking and chronic diseases,as well as serum lipid parameters in both groups. The smoking history, the CRP and FPG in the control group were significantly lower than those in the ACS group(p<0.05).(2)The content of miRNA-499 in the control group(0.792±0.313)was significantly lower than that in the ACS group(7.002±5.345),and the difference was statistically significant(P<0.05).(3)The Apo-A(1.110±0.190)and HDL(1.358±0.252)in the unstable angina subgroup were significantly higher than those in the acute myocardial infarction subgroup(1.210±0.210)(0.858±0.152).There was a statistically significant difference between the two groups(p<0.05).The CRP(2.12±2.01)and FPG(6.21±2.01)in the unstable angina group were lower than those in the acute myocardial infarction group(23.61±21.62)(9.50±5.32),with statistical differences(p<0.05).(4)The expression level of miRNA-499(7.21±5.82)in the AMI group was higher than that in the UAP group(3.29±2.73)and the control group(0.71±0.31),with statistical significance(p<0.05).Conclusion:The increased expression of plasma miRNA-499 has clinical significance in the diagnosis of acute coronary syndrome, and may be one of the important new biomarkers for judging acute myocardial infarction.
