Object The objective of this study was to determine the diagnostic value of D-di-mer in patients presenting to the emergency department (ED) with chest pain syndromes. Measurement of D-dimer may allow the screening of patients in order to identify those who most at risk of thromboembolism formation in coronary artery, and hence those who might benefit most from anti-thrombotic therapy or PTCA.Methods one hundred twenty-five consecrtive patients with acute chest pain were studied in this prospective study conducted in the ED of affiliated hospital of Qingdao medical college. D-dimer levels were measured at admission to the ED in all patients. We used correlation analysis to estimate the likelihood of myocardial infarction and the diagnostic value of D-dimer.Results D-dimer was significantly higher in patients with acute ischemic event (myocardial infarction and unstable angina )than in nonischemic patients. D-dimer level >600/g/L had an independent diagnostic value for myocardial infarction and increased the diagnostic sensitivity of the electrocardiogram , history, and cardiac enzyme from 74% to 81%.Conclusions D-dimer ,an expression of ongoing thrombus formation and lysis,is a useful marker of substantial incremental value for the early diagnosis of acute coronary syndromes presents with chest pain. It adds independent information to the diagnosis of myocardial infarction.
