Study On The Efficiency Of Government Health Expenditure In Gansu Province

In 2009,China has undergone a new round of deepening reforms in the medical and health system.The goal of the reform is to basically establish a basic medical and health system covering urban and rural residents by 2020.In the reform,it was proposed to increase government investment in health care to establish a sound public health protection system.Although Gansu Province followed the policy and put forward a series of medical and health reform policies to solve the problem of“ difficulties in accessing medical treatment and expensive medical treatment ”,there are still major problems in the health care sector.“Poverty caused by illness”and“Return to poverty due to illness”still remain Some have occurred.Although medical insurance coverage is now relatively high,urban and rural residents have introduced serious illness insurance and the proportion of secondary reimbursement has reached more than 50%.However,the net expenditure after the second reimbursement is still relatively large,and the residents' burden is heavier.This article is intended to explore the efficiency of policy implementation since the reform of Gansu Province's new health care reform,and to provide support for the reform of the policy.This paper uses DEA and Malmquist index analysis method to analyze the static and dynamic health expenditure in Gansu Province from 2010 to 2015,and uses Tobit regression to analyze the influentialfactors.The conclusions are as follows: First,the government expenditure on health in Gansu Province The overall efficiency of the performance is not satisfactory,and the city differentiation is serious,indicating that the city expenditure management is low.As the overall expenditure shows decreasing returns to scale,Gansu pays more attention to the absolute number of medical and health expenditures,with less emphasis on efficiency.The performance of technical efficiency is not satisfactory.The province's introduction of advanced technologies and human resources for health care is not enough and innovation is not enough.Second,the efficiency of Gansu provincial government's health expenditure is affected by population density,GDP per capita,total dependency ratio and health expenditure as a share of fiscal expenditure.According to the analysis of Malmquist index,overall total factor change in Gansu Province showed a downward trend,mainly due to the technical efficiency has been low.According to the empirical results of this paper,this paper proposes to optimize the structure of government health expenditure,improve the level of government health budget management and the planning level of health resources in Gansu Provinces,and implement regional management and local policies to improve the corresponding policies effectiveness.Combined with the background of grading diagnosis and treatment in the new era,the current medical and health policies in Gansu Province are optimized to narrow the gapbetween urban and rural medical and health care.
