Objective:To select a simple and practical index for clinical department efficiency evaluation,and to analyze the efficiency and variation of 25 clinical departments in a first class tertiary general hospital in Guangxi from 2014 to2018 using Data Envelopment Analysis(DEA)evaluation model.And to provide strategies and suggestions for further improvement and development.Methods: The evaluation indexes were selected according to related literature review and selection criteria.The relevant data of 25 clinical departments in sample hospital from 2014 to 2018 were collected and calculated by Excel.And DEA-CCR was analyzed by data envelopment analysis utilizing DEAP2.1 software.DEA-BCC and Malmquist models were used to analyze the efficiency and variation of 25 clinical departments in sample hospital.Results:1.Four input indexes were selected: the number of medical staffs,the number of beds,the depreciation value of fixed assets,the total expenditure,and four output indexes: outpatient and emergency patients,discharge time,rescue time,total income.The evaluation model showed operability and representativeness in efficiency evaluation of clinical departments in sample hospital.2.The overall trend of average technical efficiency in 25 clinical departments of sample hospitals during the five years from 2014 to 2018 was regressive(0.955 ~ 0.930),accounting for 28% of the effective departments.The net technical efficiency fluctuated between 0.963 and 0.984,and the overall trend decresed slightly,with 52% of the effective departments.The overall trend of scale efficiency was basically consistent with that of technical efficiency,the curve was slightly downward,with the slight fluctuation(0.951 ~0.983),and the proportion of effective departments was 32%.3.Of the 25 clinical departments in the sample hospital from 2014 to 2018,there were 8 departments with constant scale compensation in the five years,accounting for 32%,11 departments with increasing scale compensation(44%),and 5 departments with decreasing scale compensation,accounting for 20%.4.The overall TFP value of the sample hospital in 2014 to 2018 was down the curve,but there were differences in each year's TFP value,with a five-year average of 0.946.Among them,the efficiency change index first rose and then decreased,with a five-year average of 0.992;the technical change index increased year by year,with a five-year average of 0.954;the overall trend of pure technical efficiency change index was downward,with a five-year average of 0.996;and the overall trend of scale efficiency change index rose first and then decreased,with a five-year average of 0.977.Conclusion:1.From 2014 to 2018,the input and output of 25 clinicaldepartments have increased,but the overall technical efficiency,pure technical efficiency and scale efficiency have not reached the effective state,the non-optimal production mode,and each efficiency value has a downward trend,and the allocation of resources still needs to be optimized.2.From 2014 to 2018,the technical efficiency,pure technical efficiency and scale efficiency of most clinical departments did not reach the effective state,only a few departments were effective,pure technical efficiency and scale efficiency were effective,and there were differences among different departments.3.From 2014 to 2018,the total factor productivity of 25 clinical departments increased at first and then decreased,with a five-year average of0.946,which was in a "ineffective" state.Among them,the efficiency change index,the technical change index,the pure technical efficiency change index and the scale efficiency change index are all in the state of "invalid" in five years.Suggestions:1.In the hospital development planning,we should pay attention to the evaluation of hospital efficiency.2.Properly adjust the size of hospitals and departments and optimize the input elements.3.Actively introducing and New Technology to strengthen the cultivation of talents.
