Objective: To study the characteristics and clinical appearance of polymyositis(PM)or dermatomyositis(DM)combined with thyroid dysfunction or cardiac involvement,and to analyze the clinical significance,by collecting clinical data of PM/DM patients.Methods: Selecting the patients who were first diagnosed as PM/DM in Jilin university.The clinical data of all the patients were collected,including clinical manifestations,signs,laboratory test(blood routine,urine routine,myocardial enzyme,thyroid function,liver and renal function,ion,blood glucose,blood sedimentation(ESR),high-sensitivity C-reactive protein(CRP),immunoglobulin,antinuclear antibodies,electrocardiogram(ecg),echocardiography,lung CT,etc.).According to the thyroid function of the patients,all the patients were divided into normal group and abnormal group.According to the results of electrocardiogram and echocardiography of the patients,all the patients were divided into no-cardiac involvement group and cardiac involvement group.Then,the clinical characteristics were analyzed,respectively.Results: 1.When analyzing the clinical characteristics of the patients with PM/DM combined with thyroid dysfunction,there were 120 patients who were selected,including 89 patients with PM and 31 patients with DM.There were 66 cases(55.00%)in normal group and 54 cases(45.00%)in abnormal group.Among patients in abnormal group,the proportion of patients with low T3 syndrome was the largest,accounting for 15.00% with 18 cases.2.Among the patients with PM/DM combined with thyroid dysfunction,the positive rate of dysphagia,hypochromia,hematuria,proteinuria,anti-thyroid peroxidase antibody and antinuclear antibody increased and the level of ESR,CRP,creatine kinase and Ig G elevated,compared with normal group.The difference was statistically significant(P<0.05).3.When analyzing the clinical characteristics of the patients with PM/DM combined with cardiac involvement,there were 97 patients who were selected,including 71 patients with PM and 26 patients with DM.There were 39 cases(40.21%)without cardiac involvement and 58 cases(59.79%)with cardiac involvement.4.Among the patients with PM/DM combined with cardiac involvement,the positive rate of fever,edema,anti Jo-1 and antinuclear antibody increased and the level of ESR,CRP,lactic dehydrogenase elevated and the level of calcium and complement C3 reduced,compared with normal group.The difference was statistically significant(P<0.05).Conclusions: 1.In our study,the rate of the patients with PM/DM combined with thyroid dysfunction was 45.00% and the rate of patients combined with cardiac involvement was 59.79%.2.In patients with PM/DM combined with thyroid dysfunction or cardiac involvement,immune response may be more active.3.We could confer that the patients with PM/DM combined with thyroid dysfunction may be more likely to have digestive tract and kidney damaged.
